广西乡镇卫生院卫生资源配置的公平与效率研究

摘要
目的 研究广西乡镇卫生院卫生资源配置公平性及配置效率,为推进健康广西建设提供参考依据.方法 运用基尼系数分析广西乡镇卫生院各类卫生资源配置公平性,通过DEA-BCC模型和Malmquist指数分别从静态和动态的角度分析其配置效率.结果 广西乡镇卫生院机构数按乡村人口配置的基尼系数由2013 年的 0.163 上升至 2021 年的 0.199,按地理面积配置的基尼系数则由0.086下降至0.080,床位数按乡村人口、地理面积配置的基尼系数分别由2013年的0.093、0.280上升至2021年的0.106、0.312,人员数按乡村人口、地理面积配置的基尼系数分别由2013年的0.070、0.275下降至2021年的0.058、0.263;2013 年和 2021 年广西乡镇卫生院卫生资源配置效率均值均小于 1;2013 年—2021 年广西乡镇卫生院卫生资源配置的全要素生产率均值为 0.953.结论 广西乡镇卫生院卫生资源配置地理公平性相对较差,卫生资源配置效率整体欠佳,医疗技术创新能力有待优化,建议科学规划乡镇卫生院卫生资源增长,合理配置资源并提高其配置效率.
Abstract
Objective To investigate the equity and efficiency of health resource allocation in township health centers in Guangxi Zhuang Autonomous Region,so as to provide a reference for promoting the construction of"Healthy Guangxi".Methods The equity of the allocation of various types of health resources in Guangxi township health centers was analyzed using the Gini coefficient,and the static and dynamic efficiencies of the allocation were analyzed by using the DEA-BCC model and Malmquist index,respectively.Results From 2013 to 2021,the Gini coefficient of the number of township health centers by rural population increased from 0.163 to 0.199,the Gini coefficient of the number of hospitals by geographical area decreased from 0.086 to 0.080,the Gini coefficients of the number of beds by rural population and by geographical area increased from 0.093 and 0.280 to 0.106 and 0.312,respectively,and the Gini coefficients of the number of medical staff by rural population and by geographical area decreased from 0.070 and 0.275 to 0.058 and 0.263,respectively.The average health resource allocation efficiency was below 1 in 2013 and 2021.From 2013 to 2021,the average total factor productivity of health resource allocation in Guangxi township health centers was 0.953.Conclusions The equity of health resource allocation in township health centers in Guangxi is relatively poor geographically,the overall efficiency of health resource allocation is low,and the ability of medical technology innovation needs to be raised.More scientific planning of the health resource growth in township health centers may optimize the equity and efficiency of resource allocation.
