摘要

目的 研究PDCA循环管理方法在改进老年患者Ⅰ类切口手术预防用抗菌药物应用中的效果.方法 采用PDCA循环管理模式对Ⅰ类切口手术预防用抗菌药物进行管理,发现问题和分析原因后,依照PDCA循环中的4个步骤进行针对性改进.结果 经过 2 轮PDCA干预,Ⅰ类切口手术预防用抗菌药物比例从 30.47%下降到 18.63%,Ⅰ类切口手术预防用抗菌药物疗程≤24小时的百分率由79.49%上升到97.96%,Ⅰ类切口手术预防用抗菌药物时机合理率从79.49%上升到98.98%,Ⅰ类切口手术预防用抗菌药物品种选择合理率从 80.34%上升到 97.96%,差异均有统计学意义(P<0.05).结论 PDCA循环管理模式对老年患者Ⅰ类切口手术预防用抗菌药物应用的持续改进成效显著,为提高抗菌药物管理水平提供了参考依据.

Abstract

Objective To explore the effectiveness of the Plan-Do-Check-Act(PDCA)cycle in improving the prophylactic use of antibiotics in elderly patients undergoing class Ⅰ incision surgery.Methods The PDCA cycle was applied to manage the prophylactic use of antibiotics in class Ⅰ incision surgery.The identified problems were addressed in a targeted manner by following the four steps in the PDCA cycle.Results After two rounds of PDCA interventions,the rate of prophylactic antibiotic use in class Ⅰ incision surgery decreased from 30.47%to 18.63%,and the percentage of the duration of prophylactic antibiotic use for≤24 hours increased from 79.49%to 97.96%.The rate of optimal timing rose from 79.49%to 98.98%,and the rate of rational prescribing increased from 80.34%to 97.96%.Conclusions The PDCA cycle can effectively improve the prophylactic antibiotic use in class Ⅰ incision surgery,which may help to optimize antibiotics management.
