摘要
目的 探讨医疗失效模式与效应分析(healthcare failure mode and effect analysis,HFMEA)在住院患者压力性损伤质量管理中的应用,以期降低院内压力性损伤事件的发生风险,预防住院患者并发症,提高护理质量.方法 成立HFMEA小组,采用头脑风暴法对住院患者发生压力性损伤的相关因素进行分析并计算风险优先指数,针对评分较高的失效模式制定措施,实施护理质量持续改进措施后对效果进行评价,以改进和完善压力性损伤管理流程.结果 经HFMEA模式管理并采取相应措施后,住院患者压力性损伤发生率由0.018％下降至0.008％,各季度压力性损伤管理质量符合率亦逐步上升.结论 HFMEA的应用能有效提高住院患者压力性损伤的风险评估水平,有效降低住院患者压力性损伤的发生率.
Abstract
Objective To investigate the application of healthcare failure mode and effect analysis(HFMEA)in the quality management for pressure injury in inpatients,with an attempt to reduce the risk of pressure injury events,prevent complications,and improve the quality of nursing during hospital stay.Methods An HFMEA team was established.Factors related to pressure injury in inpatients were explored by brainstorming,and the risk priority numbers were calculated.For failure modes with high scores,continuous nursing quality improvement measures were implemented and assessed to optimize the pressure injury management process.Results After the HFMEA mode was applied and corresponding measures were taken,the incidence of pressure injury in inpatients decreased from 0.018％to 0.008％,and the quality compliance rate of pressure injury management increased gradually in each quarter.Conclusion The application of HFMEA can effectively improve the risk assessment for pressure injury in inpatients and reduce the incidence of pressure injury.
