三级综合医院死因监测全流程管理实践

摘要
目的 探讨医院高质量发展背景下三级综合医院死因监测工作的全流程管理实践,并分析实践效果,以期提升医院在公共卫生领域的精细化管理水平.方法 通过分析死因监测工作的发起、填报、审核、上报等各环节的全流程干预,采取完善工作制度、细化规范流程、提升业务水平及信息化应用等措施,对死因监测进行全流程管理,杜绝漏报和迟报,保证死因信息的完整和准确,提高死因监测规范化水平.结果 死因监测实行全流程管理后,一方面提升了死因监测工作的流畅性,确保了死因监测工作能够有序开展;另一方面大幅度提升了死因监测的工作质量,无死亡漏报情况发生,死因监测报卡及时率、多死因链比例分别由2022年的90.68%、60.56%上升至2023年的98.41%、69.75%,根本死因不准确比例则由2022年的13.66%下降至2023年的6.69%.结论 死因监测全流程管理模式能够有效降低迟报、漏报风险,提高死因监测规范化水平,有助于提升管理水平,对其他医院开展死因监测工作具有借鉴意义.
Abstract
Objective To investigate the whole-process management mode for mortality surveillance in a tertiary general hospital within the framework of advancing hospital high-quality development and its practice efficiency,aiming to improve the refined public health management in hospitals.Methods Through a systematic analysis of each link in the mortality surveillance process,from initiation to data entry,review,and reporting,a range of interventions were implemented in the whole-process management mode,including refining the operational framework,standardizing procedures,improving professional expertise,and leveraging information technology to ensure comprehensive management of mortality surveillance,avoid underreporting and delayed reporting,and guarantee the completeness and accuracy of mortality data to promote the standardization of mortality surveillance practices.Results The adoption of the whole-process management mode enabled the mortality surveillance to be carried out in a smooth and orderly manner,and remarkably raised working quality.No deaths were missed in the reporting.The promptness rates of report for mortality surveillance and the proportion of multiple causes of death(MCOD)increased from 90.68%and 60.56%in 2022 to 98.41%and 69.75%in 2023,respectively,while the proportion of inaccurate underlying cause of death(UCOD)decreased from 13.66%in 2022 to 6.69%in 2023.Conclusion The whole-process management model for mortality surveillance can effectively reduce delayed and missed reporting,foster the standardization of mortality surveillance,and raise management levels.
