摘要
目的 探索静脉用药调配中心(Pharmacy Intravenous Admixture Services,PIVAS)高警示药品的管理方法,减少调配差错的发生.方法 采取的措施包括制定高警示药品管理制度与目录,规范药品请领、储存、标识,加强新进药品、量化脱包管理及高警示药品日盘点,加强人员培训,完善信息系统及优化工作流程.比较加强高警示药品管理实施前后的调配差错发生情况.结果 加强高警示药品管理后,高警示药品调配差错发生率由0.64％o.下降至0.16‰,差异具有统计学意义(P＜0.05).结论 制定的措施切实可行,确保了高警示药品的使用安全.
Abstract
Objective To investigate and optimize management strategies for high-alert medications(HAMs)in Pharmacy Intravenous Admixture Services(PIVAS)with the aim of diminishing dispensing errors.Methods A comprehensive system and catalog of HAMs were developed to standardize drug reception,storage,and labeling.Additionally,management of new medications,off-package quantity control,and daily inventory of HAMs were enhanced.Staff training initiatives were intensified,the information system was refined,and workflow processes were optimized.The impact of these interventions on HAM management was assessed by comparing outcomes before and after the implementation.Results The rate of HAM dispensing errors decreased from 0.64 per 10,000 to 0.16 per 10,000 following the implementation of enhanced management practices(P＜0.05).Conclusion The implemented measures are viable and effective in ensuring the safety of HAMs.
