目的 探讨孕早期外泌体微小RNA(miR)-876-3p与妊娠糖尿病(GDM)和胰岛素抵抗(IR)、脂联素(ADP)水平的关系.方法 选取2021年1月1日-12月31日期间在西北妇女儿童医院产科接受常规产前检查并分娩的150例孕产妇作为研究对象,包括50例GDM孕妇(GDM组)和100例无GDM的正常孕妇(非GDM组).提取并纯化受试者循环外泌体,通过实时荧光定量PCR法检测受试者孕早期循环外泌体miR-876-3p表达水平.结果 GDM组孕早期循环外泌体miR-876-3p、糖化血红蛋白(HbA1c)、空腹胰岛素(Fins)、稳态模型-胰岛素抵抗指数(HOMA-IR)、瘦素(LEP)水平均显著高于非GDM组,ADP显著低于非GDM组(P<0.05).ROC曲线分析循环外泌体miR-876-3p表达预测GDM的曲线下面积(AUC)为0.806(95%CI:0.734~0.878).在将循环外泌体miR-876-3p添加到经典的GDM风险因素模型中后,可显著增加经典预测模型的预测效能.经Spearman秩相关性分析,循环外泌体miR-876-3p表达与HbA1c、Fins、HOMA-IR、LEP呈正相关,与ADP呈负相关(P<0.05).结论 孕早期循环外泌体miR-876-3p水平高表达与GDM高风险以及IR加重和ADP水平降低有关.循环外泌体miR-876-3p有希望成为孕早期GDM临床筛查的生物标志物以及潜在的治疗靶点.
Relationship between early pregnancy exosomal miR-876-3p and gestational diabetes mellitus,insulin resistance and adiponectin levels
Objective To investigate the relationship between exosomal microRNA(miR)-876-3p and gestational diabetes mellitus(GDM),insulin resistance(IR)and adiponectin(ADP)levels in early pregnancy.Methods A total of 150 pregnant and postpartum women who underwent routine prenatal examinations and delivered in the obstetrics department of Northwest Women's and Children's Hospital from January 1 to December 31,2021 were selected as the research subjects,including 50 pregnant women with GDM(GDM group)and 100 normal pregnant women without GDM(non-GDM group).The circulating exosomes of the subjects were extracted and purified,and the expression level of miR-876-3p in the circulating exosomes of the subjects in the first trimester was detected by real-time fluorescence quantitative PCR.Results The levels of circulating exosomal miR-876-3p,glycated hemoglobin(HbA1c),fasting insulin(Fins),homeostasis model-insulin resistance index(HOMA-IR)and leptin(LEP)in early pregnancy in GDM group were significantly higher than those in non-GDM group,and ADP was significantly lower than that in non-GDM group(P<0.05).ROC curve analysis of circulating exosome miR-876-3p expression predicted GDM with an area under the curve(AUC)of 0.806(95%CI:0.734-0.878).After adding circulating exosome miR-876-3p to the classical GDM risk factor model,it could significantly increase the predictive per-formance of classic prediction models.By Spearman rank correlation analysis,the expression of circulating exosomal miR-876-3p was positively correlated with HbA1c,Fins,HOMA-IR and LEP,and negatively correlated with ADP(P<0.05).Conclusion The high expression of circulating exosomal miR-876-3p in early pregnancy is associated with the high risk of GDM,as well as the aggravation of IR and the decrease of ADP level.It is promising to become a biomarker for clinical screening of GDM in early pregnancy and a potential therapeutic target.
