Relationship between early pregnancy exosomal miR-876-3p and gestational diabetes mellitus,insulin resistance and adiponectin levels
Objective To investigate the relationship between exosomal microRNA(miR)-876-3p and gestational diabetes mellitus(GDM),insulin resistance(IR)and adiponectin(ADP)levels in early pregnancy.Methods A total of 150 pregnant and postpartum women who underwent routine prenatal examinations and delivered in the obstetrics department of Northwest Women's and Children's Hospital from January 1 to December 31,2021 were selected as the research subjects,including 50 pregnant women with GDM(GDM group)and 100 normal pregnant women without GDM(non-GDM group).The circulating exosomes of the subjects were extracted and purified,and the expression level of miR-876-3p in the circulating exosomes of the subjects in the first trimester was detected by real-time fluorescence quantitative PCR.Results The levels of circulating exosomal miR-876-3p,glycated hemoglobin(HbA1c),fasting insulin(Fins),homeostasis model-insulin resistance index(HOMA-IR)and leptin(LEP)in early pregnancy in GDM group were significantly higher than those in non-GDM group,and ADP was significantly lower than that in non-GDM group(P<0.05).ROC curve analysis of circulating exosome miR-876-3p expression predicted GDM with an area under the curve(AUC)of 0.806(95%CI:0.734-0.878).After adding circulating exosome miR-876-3p to the classical GDM risk factor model,it could significantly increase the predictive per-formance of classic prediction models.By Spearman rank correlation analysis,the expression of circulating exosomal miR-876-3p was positively correlated with HbA1c,Fins,HOMA-IR and LEP,and negatively correlated with ADP(P<0.05).Conclusion The high expression of circulating exosomal miR-876-3p in early pregnancy is associated with the high risk of GDM,as well as the aggravation of IR and the decrease of ADP level.It is promising to become a biomarker for clinical screening of GDM in early pregnancy and a potential therapeutic target.
