摘要

目的 了解衡水市新生儿先天性甲状腺功能减低症(CH)的筛查、诊断、治疗、随访情况及需要改进的问题.方法 对衡水市2004-2022年进行筛查的797431例新生儿资料进行回顾性分析总结.结果 797431例新生儿中疑似患儿423例,经静脉采血检测甲状腺功能及超声检测确诊CH 273例,高TSH血症27例,发病率为1:2920,273例CH患儿中甲状腺缺失、发育不全或异位82例占30.0%,甲状腺发育正常191例占70.0%.273例CH患儿均得到及时的早期诊断和治疗,高TSH血症患儿定期检测甲状腺功能.结论 通过筛查让患病新生儿能够获得及时、有效的诊治,经过跟踪随访与实际情况相结合发现其中存在的问题,对改善出生人口质量提供保障的同时更好地推进了新生儿疾病筛查工作的进展.

Abstract

Objective To investigate the screening,diagnosis,treatment and follow-up of neonatal congenital hypo-thyroidism(CH)in Hengshui city and the problems that need to be improved.Methods The data of 797431 neonates screened from 2004 to 2022 in Hengshui city need to be retrospectively analyzed and summarized.Results There were 423 suspected children among 797431 neonates.273 cases of CH were confirmed by intravenous blood collection and ultrasonography,27 cases were with high TSH,and the incidence was 1:2920.Among 273 children with CH,82 cases were thyroid deletion,hy-poplasia or ectopic,accounting for 30.0%,and 191 cases were with normal thyroid development,accounting for 70.0%.All 273 children with CH received timely early diagnosis and treatment,and thyroid function was detected regularly in children with high TSH.Conclusion Through the screening of neonatal congenital hypothyroidism,the children can get timely and effective treatment.After follow-up combined with the actual situation,the existing problems were found.And it provided a guarantee for improving the quality of the birth population.At the same time,it also better promoted the development and promotion of newborn disease screening.
