Effect of micro-density-gradient centrifugation on sperm morphology,activity rate,DNA fragmentation rate and clinical pregnant outcome of intrauterine insemination

目的 评价微量梯度离心分离精子质量及对宫腔内人工授精(IUI)结局影响.方法 分别用0.5mL(微量法)和1 mL 90%/45%(常规法)双层梯度离心分离60份精液,分析处理前后及不同组精子密度、活动率、正常形态率、自发顶体反应(AR)率、DNA碎片指数(DFI)和精子DNA高染率(HDS).卡方检验统计两种方法对IUI结局影响.结果 微量法精子回收率高于常规法(P=0.000).微量法前向运动精子百分率低于常规法(P=0.047),高于处理前(P=0.000、0.000).微量法DFI、HDS与常规法比较无差异(P=0.217、0.417),与处理前相比也没有显著性差异(P=0.498、0.993、0.604、0.465).微量法正常形态率与常规法比较无差异(P=0.470),高于处理前(P=0.000).微量法自发AR率与常规法比较无差异(P=0.244).微量法回收精子行IU1显著提高IUI临床妊娠率(x2=6.192,P=0.022).结论 微量法提高回收率和IUI临床妊娠率.
Objective To evaluate the effect of micro-density-gradient centrifugation(micro-DGC)on the quality of sperm and IUI pregnant outcome.Methods 60 semen specimens underwent 0.5 mL two layer(90%/45%)and 1 mL(90%/45%)density gradient centrifugation respectively.The sperm density,motility,normal sperm morphology rate,the spontaneous acrosomal reaction rate,sperm DNA fragmentation rate and HDS before and after treatment were analyzed.The effect of two different methods on IUI pregnant outcome was assessed by chi-square test.Results After separation,the sperm recovery rate of the micro-DGC was significantly higher than that of routine-DGC(P=0.000).The ratio of motile sperm mount and total sperm mount of micro-DGC is lower than that of routine-DGC,and there was significantly statistical difference(P=0.047).The percentages of motile sperm of routine and micro-DGC were significantly higher than that of pre-treatment(P=0.000,0.000).There was no statistical difference(P=0.217,0.417)between two groups for the sperm DFI rate and HDS rate.Mean-while,the DFI rate and HDS rate after micro and routine-DGC methods were no higher than that of pre-treatment,(P=0.498,0.993,0.604,0.465).Compared with pre-treatment,the micro and routine-DGC method significantly improved the normal morphological rate(P=0.000),yet,there was no significant difference between two groups(P=0.470).No statistical difference(P=0.244)was shown between two groups for spontaneous AR rate.The clinical pregnant rate of IUI significantly improved by using micro-DGC method(x2=6.192,P=0.022),compared with routine-DGC method.Conclusion The sperm recovery rate improved by use of micro-DGC,and the outcome of IUI by use of micro-DGC was better.
