通过PDCA循环提高产前超声估测胎儿的体重阳性预测值

摘要
目的 通过PDCA循环提高产前超声估测胎儿体重阳性预测值.方法 随机调取保定市妇幼保健院2022年9-12月期间足月出生的486例病例资料,定义分娩前72h超声评估胎儿体重与新生儿实际出生体重误差的绝对值≥250 g为阳性.阳性预测值指产前超声预测的阳性例数中,真正"偏大或偏小"的例数,即真阳性所占的比例.本组病例产前72 h超声预测阳性52例,其中真阳性22例,阳性预测值42％.选取2023年1月1日至4月30日作为PDCA循环期间.结果 PDCA循环实施后,超声估测胎儿体重阳性预测值提升至52％,提高了 10％.结论 本次PDCA循环有效提高了产前超声预测胎儿体重的阳性预测值.
Abstract
Objective Improve the positive predictive value of prenatal ultrasound fetal weight estimation through PDCA cycle.Methods Data of 486 cases of full-term birth in Baoding Maternal and Child Health Hospital from September to December 2022 were randomly selected.The absolute value of the difference between the estimated fetal weight by ultrasound evaluation within 72 hours before delivery and the actual birth weight of newborns≥250 g is considered positive.The positive predictive value refers to the proportion of truly"overestimated or underestimated"cases in prenatal ultrasound predictions among the positive cases,that is,the proportion occupied by true positives.Among the cases in this group,there were 52 positive results predicted by ultrasound within 72 hours before delivery.Among them,there were 22 true positive cases,with a positive predictive value of 42％.January 1,2023 to April 30,2023 was selected as the period for the PDCA cycle.Results After imple-menting the PDCA cycle,the positive predictive value of ultrasound estimated fetal weight increased to 52％,an improvement of 10％.Conclusion This PDCA cycle effectively improved the positive predictive value of prenatal ultrasound for estimating fetal weight.
