血清CCL28、SDF-1水平与慢性阻塞性肺疾病合并肺动脉高压患者病情及预后的关系

目的 探讨血清CC趋化因子配体28(CCL28)、基质细胞衍生因子-1(SDF-1)水平与慢性阻塞性肺疾病(COPD)合并肺动脉高压患者病情及预后的关系。方法 选取济宁医学院附属医院2019年1月至2023年6月收治的98例COPD合并肺动脉高压患者作为病例A组,选取100例不合并肺动脉高压的COPD患者作为病例B组,100例健康体检志愿者作为对照组。对比三组的血清CCL28、SDF-1水平,并分析COPD合并肺动脉高压患者的血清CCL28、SDF-1水平与肺动脉压参数、肺功能指标的相关性,对比COPD合并肺动脉高压患者治疗前后的各项指标变化。结果 病例A组的血清CCL28、SDF-1水平显著高于病例B组、对照组,差异均有统计学意义(均P<0。05);病例B组的血清CCL28、SDF-1水平显著高于对照组,差异均有统计学意义(均P<0。05)。病例A组患者的平均肺动脉压(mPAP)、肺动脉楔压、肺血管阻力、动脉血二氧化碳分压(PaCO2)、残总气量百分比(RV/TLC)高于病例B组,动脉血氧分压(PaO2)、第一秒用力呼气容积占预计值的百分比(FEV1 pred%)、肺一氧化碳弥散量(DLCO)低于病例B组,差异均有统计学意义(均P<0。05)。病例A组患者的血清CCL28、SDF-1水平与mPAP、肺动脉楔压均呈正相关(均P<0。05),与FEV1 pred%、RV/TLC、DLCO无关(均P>0。05)。病例A组患者治疗后的血清CCL28、SDF-1水平及mPAP、肺动脉楔压、肺血管阻力、PaCO2、RV/TLC较治疗前显著降低,Pa02、FEV1 pred%、DLCO较治疗前显著升高,差异均有统计学意义(均P<0。05)。结论 COPD合并肺动脉高压患者的血清CCL28、SDF-1水平显著升高,并且与肺动脉高压病情程度密切相关,积极治疗后患者的血清CCL28、SDF-1水平会显著降低。
Relationship between the serum CCL28 and SDF-1 levels and the condition and prognosis of chronic obstructive pulmonary disease complicated with pulmonary hypertension
Objective To investigate the relationship between the expression levels of CC chemokine ligand 28(CCL28)and stromal cell derived factor-1(SDF-1)in serum and the condition and therapeutic effect of chronic obstructive pulmonary disease(COPD)complicated with pulmonary hypertension.Methods A total of 98 COPD patients with pulmonary hypertension admitted to the Affiliated Hospital of Jining Medical College from January 2019 to June 2023 were selected as case group A,100 COPD patients without pulmonary hypertension were selected as case group B,and 100 healthy volunteers were selected as control group.The serum levels of CCL28 and SDF-1 in the three groups were compared,and the correlation between the serum levels of CCL28 and SDF-1 in patients with COPD combined with pulmonary hypertension and pulmonary artery pressure parameters and pulmonary function indexes was analyzed,and the changes of various indexes in patients with COPD combined with pulmonary hypertension were compared before and after treatment.Results The levels of CCL28 and SDF-1 in the case group A were significantly higher than those in the case group B and the control group,with statistical significance(all P<0.05).The levels of CCL28 and SDF-1 in the case group B were significantly higher than those in the control group,and the differences were statistically significant(all P<0.05).The mean pulmonary artery pressure(mPAP),pulmonary artery wedge pressure,pulmonary vascular resistance,arterial partial pressure of carbon dioxide(PaCO2)and percentage of residual total gas volume(RV/TLC)in the case group A were higher than those in the case group B.The partial pressure of arterial blood oxygen(PaO2),the percentage of forced expiratory volume in the first second to the predicted value(FEV,pred%),and the pulmonary carbon monoxide dispersion(DLCO)were lower than those of case group B,and the differences were statistically significant(all P<0.05).Serum CCL28 and SDF-1 levels in the case group A were positively correlated with mPAP and pulmonary wedge pressure(all P<0.05),but not with FEV,pred%,RV/TLC and DLCO(all P>0.05).After treatment,the serum levels of CCL28 and SDF-1,mPAP,pulmonary wedge pressure,pulmonary vascular resistance,PaCO2 and RV/TLC in case group A were significantly decreased compared with before treatment,while PaO2,FEV,pred%and DLCO were significantly increased compared with before treatment,with statistical significance(all P<0.05).Conclusions The serum levels of CCL28 and SDF-1 in patients with COPD combined with pulmonary hypertension are significantly increased,and are closely related to the severity of pulmonary hypertension.After active treatment,the serum levels of CCL28 and SDF-1 in patients will be significantly reduced.
