Effect of Antibiotics Combined with Dexamethasone in the Treatment of Preterm Premature Rupture of Membranes and its Effect on Neonatal Outcome
Objective:To investigate the effect of antibiotics combined with dexamethasone in the treatment of preterm premature rupture of membranes(PPROM)and its effect on neonatal outcomes.Methods:A total of 122 pregnant women with PPROM admitted to the Third Affiliated Hospital of Wenzhou Medical University from January to August 2022 were divided into control group and observation group by random number table method,with 61 pregnant women in each group.The control group was treated with antibiotics+conventional treatment,and the observation group was treated with dexamethasone on the basis of the control group.The course of treatment was 7 days.The serum indexes[white blood cell(WBC),C-reactive protein(CRP),procalcitonin(PCT)],maternal pregnancy and outcome,neonatal outcome and adverse drug reactions were compared between the two groups.Results:After treatment,the serum WBC in the observation group was higher than that in the control group,while the levels of CRP and PCT were lower than those in the control group(P<0.05).The time from rupture of membranes to delivery in the observation group was longer than that in the control group,and the body temperature before delivery was lower than that in the control group(P<0.05).The incidence of HCA,cesarean section,postpartum hemorrhage and puerperal infection in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the incidence of postpartum hemorrhage between the two groups(P>0.05).The incidence of neonatal fetal distress,neonatal pneumonia and neonatal respiratory distress syndrome in the observation group were lower than those in the control group(P<0.05),but there was no significant difference in the incidence of neonatal death and early-onset sepsis between the two groups(P>0.05).There was no significant difference in the incidence of drug-related adverse reactions between the two groups(P>0.05).Conclusion:Antibiotics combined with dexamethasone can effectively improve the serum indexes of PPROM parturients,prolong the time from membrane rupture to delivery,reduce the infection after membrane rupture,improve the maternal and infant outcomes,and have good safety.
