Comparison of the Effects of Different Doses of Methylprednisolone Combined with Azithromycin in the Treatment of Adult with Refractory Mycoplasma Pneumonia
Objective:To observe the effect of different doses of methylprednisolone combined with azithromycin in aldult with refractory mycoplasma pneumoniae pneumonia(RMPP).Methods:From September 2021 to August 2023,198 aldult with RMPP who were treated in Zigui County Traditional Chinese Medicine Hospital were randomly separated into group A(n=66),group B(n=66),and group C(n=66)using a random number table method.All three groups were treated with azithromycin,on this basis,group A received low-dose[1 mg/(kg·d)]methylprednisolone treatment,group B received medium dose[2 mg/(kg·d)]methylprednisolone treatment,and group C received high-dose[5 mg/(kg·d)]methylprednisolone treatment.The efficacy,the symptom improvement time,the levels of serum inflammatory indicators[interleukin-6(IL-6),erythrocyte sedimentation rate(ESR),interferon-γ(IFN-γ)and white blood cell(WBC)],blood gas analysis indicators[arterial oxygen partial pressure(PaO2),carbon dioxide partial pressure(PaCO2)]and adverse drug reactions(ADRs)were compared among three groups.Results:The total effective rate of group C was 93.94%,which was higher than 78.79%and 81.82%of group A and group B(P<0.05).The disappearance time of cough,body temperature normalization and pulmonary rales in group C was shorter than that in group A and group B(P<0.05).After 10 days of treatment,the levels of serum IL-6,ESR,IFN-γ,WBC and PaCO2 in group C were lower than those in group A and group B,and PaO2 was higher than that in group A and group B(P<0.05).There was no significant difference in the total incidence of ADR among group A,group B and group C(P>0.05).Conclusion:The combination of high-dose methylprednisolone and azithromycin has a better therapeutic effect on aldult with RMPP,which can effectively improve inflammation status,blood gas analysis indicators,obviously shorten the time of symptom disappearance,and have a lower ADR.
