复方葛根芩连汤联合泼尼松治疗肺纤维化的临床研究

摘要
目的:研究复方葛根芩连汤联合泼尼松对肺纤维化的疗效.方法:选择2021年1月—2023年12月我院收治的70 例肺纤维化患者,随机分为对照组和观察组,每组35 例.对照组采用泼尼松治疗,观察组采用复方葛根芩连汤联合泼尼松治疗.比较两组症状评分、肺功能[最大呼气压(PEmax)、用力肺活量(FVC)、呼气峰流速(PEF)、一氧化碳弥散量(DLCO)、最大吸气压(PImax)]和炎症因子[血清肿瘤坏死因子-α(TNF-α)、转化生长因子-β(TGF-β)].结果:治疗后,观察组呼吸困难、咳嗽、乏力、发绀和咳痰评分低于对照组(P<0.05);观察组PEmax、FVC、PEF、DLCO和PImax高于对照组(P<0.05);观察组血清TNF-α和TGF-β水平低于对照组(P<0.05).结论:复方葛根芩连汤联合泼尼松能明显减轻肺纤维化患者的症状,改善其肺功能,其机制可能与下调TGF-β水平有关.
Abstract
Objective:To study the curative effect of compound Gegen Qinlian decoction combined with prednisone on pulmonary fibrosis.Methods:A total of 70 patients with pulmonary fibrosis admitted to our hospital from January 2021 to December 2023 were randomly divided into control group and observation group,with 35 cases in each group.The control group was treated with prednisone,and the observation group was treated with compound Gegen Qinlian decoction combined with prednisone.The symptom scores,lung function[maximum expiratory pressure(PEmax),forced vital capacity(FVC),peak expiratory flow(PEF),carbon monoxide diffusion(DLCO),maximum inspiratory pressure(PImax)]and inflammatory factors[serum tumor necrosis factor-α(TNF-α),transforming growth factor-β(TGF-β)]were compared between the two groups.Results:After treatment,the scores of dyspnea,cough,fatigue,cyanosis and expectoration in the observation group were lower than those in the control group(P<0.05).The PEmax,FVC,PEF,DLCO and PImax in the observation group were higher than those in the control group(P<0.05).The levels of serum TNF-α and TGF-β in the observation group were lower than those in the control group(P<0.05).Conclusion:Compound Gegen Qinlian decoction combined with prednisone can significantly reduce the symptoms of patients with pulmonary fibrosis and improve their lung function.The mechanism may be related to the down-regulation of TGF-β level.
