Lipid compliance rate and influencing factors of ultra-high-risk patients with atherosclerotic cardiovascu-lar diseases
Objective lo evaluate the lipid compliance rate and influencing factors of ultra-high-risk patients with atherosclerotic cardiovascular disease(ASCVD).Methods Ultra-high-risk patients with ASCVD who were hospitalized in the Inner Mongolia Autonomous Region People's Hospital from January 2020 to December 2020 were selected as study subjects,and patient datas were collected through the PASS clinical pharmacist management system.The included indicators included laboratory data at the time of patient's admission to the hospital and at the time of review within 3 weeks to 6 months after discharge,including low-density lipoprotein cholesterol(LDL-C),triglycerides(TG),total cholesterol(TC),and high-density lipoprotein cholesterol(HDL-C).For the collected data,the chi-square tests were used to compare the baseline information of patients in the lipid-compliance group and the non-compliance group,and univariate and multivariate logistic regression analyses were conducted to explore the factors affecting lipid attainment in ultra-high-risk patients with ASCVD.Results A total of 214 patients with ultra-high risk of ASCVD were included in this study,and 41 of them achieved the target lipid levels after treatment with lipid-lowering drugs,with an attainment rate of 19.2%(41/214).For the baseline data of the lipid-compliance group and the lipid-non-compliance group of patients with different clinical characteristics,the results showed that there were significant differences in statin intensity,history of myocardial infarction(MI),and history of percutaneous coronary intervention(PCI),and the differences were statistically significant(P<0.05).The results of the univariate logistic regression analysis showed that the history of MI[OR(95%CI):2.242(1.101,4.566)],history of PCI[OR(95%CI):2.178(1.043,4.546)]and use of high-intensity statins[OR(95%CI):3.361(1.509,7.483)]were the influencing factors for patients'lipid attainment(P<0.05).And multifactorial logistic regression analysis showed that high-intensity statin[OR(95%CI):2.805(1.211,6.497)]use was an influential factor in patients'lipid compliance(P<0.05).Conclusion The lipid compliance rate among the ultra-high-risk ASCVD patients is relatively low,and high-intensity statin therapy can significantly improve their lipid attainment rate.
