Short-term effect of sodium alginate oral water gel on periodontal status in elderly patients with smoking periodontitis
Objective To investigate the short-term effect of sodium alginate oral water gel on periodontal status and levels of soluble intercellular adhesion molecule 1(sICAM-1)and interleukin-1β(IL-1β)in gingival crevicular fluid in elderly patients with smoking periodontitis. Methods A total of 152 elderly patients with smoking periodontitis admitted to Hangzhou Xiaoshan Hospital of Traditional Chinese Medicine from August 2020 to October 2022 were selected as the study subjects.According to different treatment methods, they were divided into two groups:control group(76 cases)was treated with iodine glycerin after subgingival curettage, and observation group(76 cases)was treated with sodium alginate oral water gel after subgingival curettage.Periodontal indexes[gingival index(GI),gingival sulcus bleeding index(SBI),plaque index(PLI),periodontal pocket depth(PD)and attachment loss(AL)],gingival sulcus fluid sICAM-1,IL-1β,IL-6,TNF-α,the incidence of adverse reactions and short-term recurrence rate before and after treatment were measured and statistically analyzed. Results GI,SBI,PLI,PD and AL of the two groups were decreased significantly after treatment(P<0.001).GI,SBI,PLI,PD and AL of the observation group were significantly lower than those of the control group, and the difference was statistically significant(t=10.450,6.295,7.372,14.454,7.280,P<0.01);After treatment, the levels of sICAM-1,IL-1β,IL-6 and TNF-α in gingival crevicular fluid of 2 groups were significantly decreased(P<0.01).The levels of sICAM-1,IL-1β,IL-6 and TNF-α in gingival crevicular fluid of the observation group were significantly lower than those of the control group, and the difference was statistically significant(t=8.993,9.684,10.184,5.664,P<0.01).There was no significant difference in the incidence of adverse reactions between 2 groups(P>0.05).After 3 months of follow-up,the loss of follow-up rate was 3.3%,and the recurrence rate in the observation group was significantly lower than that in the control group(1.3%vs 9.6%),and the difference between the two groups was statistically significant(χ2=4.846,P=0.028).Conclusion Sodium alginate oral water gel can effectively improve the periodontal status of elderly patients with smoking periodontitis, inhibit inflammation in deep pockets, and reduce the short-term recurrence rate, which is worthy of clinical application.
