高度近视合并白内障区域折射多焦点人工晶状体植入术后视觉质量的观察

目的 观察高度近视合并白内障患者植入区域折射多焦点人工晶状体(MIOL)术后的视觉质量和在囊袋内的稳定性。方法 选取西安市人民医院2019年7月至2020年6月行超声乳化吸除联合人工晶状体植入手术的患者37例45眼,分为3组,A组为高度近视合并白内障植入区域MIOL患者11例(15眼),B组为高度近视合并白内障植入单焦点人工晶状体(SIOL)患者13例(15眼),C组为单纯老年性白内障植入区域MIOL患者13例(15眼)。术后观察3组的1个月、3个月和6个月的远近视力、视觉质量和STOL在囊袋内的倾斜性。结果 45眼术后A组、B组和C组的裸眼远视力(UDVA)均较术前提高,A组和C组的UDVA和裸眼近距离视力(UNVA)较B组提高,差异有统计学意义(P<0。05);A组、B组和C组术后的调制传递函数截止频率(MTF cutoff)均较术前提高,客观散射指数(OSI)较术前下降,差异均有统计学意义(P<0。05)。组间比较,术后A组、C组MTF cutoff较B组小,OSI较B组大,差异无统计学意义(P>0。05)。术后A组、C组全眼的总高阶相差、眼内慧差、三叶草、球差均比B组增加,尤其表现为三叶草增加明显,差异有统计学意义(P<0。05)。术后3个月A组、B组和C组的倾斜性[Tilt(S1)]值在6 mm瞳孔下均小于0。6,3组差异无统计学意义(P>0。05)。结论 区域MIOL植入高度近视合并白内障患者可提供良好的远、近视力,提高患者视觉质量,脱镜率高,早期囊袋内稳定好,但术后全眼的高阶像差增加,主要表现为三叶草增加。
Observation of visual quality after multifocal intraocular lens implantation in patients with high myopia complicated with cataracts
Objective To observe the visual quality and stability in the capsular bag of patients with high myopia complicated with cataract after they received multi-focal intraocular lens implantation.Methods A total of 45 patients who underwent phacoemulsification combined with intraocular lens implantation surgery from July 2019 to June 2020 in Xi'an People's Hospital were selected and divided into 3 groups.Group A contained 11 patients(15 eyes)with high myopia complicated with cataracts who were implanted with multifocal intraocular lens(MIOL).Group B contaied 13 patients(15 eyes)]with high myopia complicated with cataracts who were implanted with single-focus intraocular lens(SIOL).Group C Contained 13 patients(15 eyes)with simple senile cataracts who were implanted with multifocal intraocular lens(MIOL).After the surgery,the far and near vision,visual quality,and the tilt of the intraocular lens in the capsular bag of the three groups were observed at 1 month,3 months,and 6 months.Results The UDVA of the 45 eyes in group A,B,and C were all significantly improved after surgery.The UDVA and UNVA in group A and C were significantly improved compared with group B,and the difference was statistically significant(P<0.05).After surgery,the MTF of group A,B,and C were significantly improved,while the OSI value was significantly decreased,and the differences were statistically significant(P<0.05).In the comparison among groups,the MTF of group A and C after surgery were smaller than that of group B,and OSI was larger than that of group B,but the difference was not statistically significant(P>0.05).The total high-order aberra-tions,intraocular coma,trefoil,and spherical aberrations of the whole eye in group A and C after surgery were all increased compared with group B,especially the trefoil was significantly increased,and the difference was statisti-cally significant(P<0.05).Three months after surgery,the Tilt(S1)values of group A,B,and C under a 6 mm pupil were all less than the normal range of 0.6,and there was no significant difference among the three groups(P>0.05).Conclusion Implantation of regional multifocal intraocular lens for patients with high myopia complicat-ed with cataracts can provide good far and near vision,improve patients'visual quality,and have a high rate of spectacle independence.It has good early stability in the capsular bag,but the total high-order aberration of the whole eye after surgery increases,mainly manifesting as an increase in trefoil.
