Effects of esketamine and sufentanil combined anesthesia on hemodynamics and plasma nitric oxide and endothelin-1 in patients undergoing laparoscopic surgery
Objective To investigate the effects of esketamine and sufentanil combined anesthesia on hemodynamics and plasma nitric oxide(NO)/endothelin-1(ET-1)in patients undergoing laparoscopic surgery. Methods A total of sixty patients who underwent elective laparoscopic cholecystectomy(ASA grade Ⅰ~Ⅱ)in Cangnan County Hospital from June 2022 to May 2023 were enrolled in this study.According to the random num-ber table method, they were divided into an etomidate group and a sufentanil group, with 30 cases in each group. The esketamine group received esketamine combined anesthesia, and the sufentanil group received sufentanil combined anesthesia.Compare the diastolic blood pressure(DBP),systolic blood pressure(SBP),heart rate(HR),arterial blood carbon dioxide partial pressure(PaCO2),plasma levels of NO and ET-1,as well as the occurrence of adverse reactions between the two groups at different time points:before anesthesia(T1),before pneumoperitoneum(T2),20 minutes after pneumoperitoneum(T3),and 20 minutes after the release of pneumoperitoneum(T4).Results At T,and T4,there was no statistically significant difference in DBP,SBP,PaCO2,and HR between the two groups(P>0.05).At T2 and T3,both DBP and HR levels in the two groups were lower/slower than at T1.In the esketamine group, the HR was faster and the PaCO2 was lower compared to the sufentanil group(P<0.05).At T1,there was no statistically significant difference in plasma NO and ET-1 levels between the two groups(P>0.05).At T2,T3,and T4,the plasma NO level in the esketamine group was lower than in the sufentanil group, while the ET-1 level was higher than in the sufentanil group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Compared to the sufentanil combined anesthesia, esketamine can better maintain the hemodynamic stability and the balance of plasma NO/ET-1 in patients under-going laparoscopic cholecystectomy, without increasing the incidence of adverse reactions. Therefore,it is worthy of promotion and application.
