Establishment of D-dimer reference interval for pregnant women in different pregnancy periods
Objective To explore and establish the reference interval of D-dimer(D-D)in different pregnancy periods of pregnant women,so as to provide more accurate reference for the diagnosis of thrombotic diseases and the intervention of anticoagulant drugs.Methods A total of 140 pregnant women who underwent prenatal examination in the Second People's Hospital of Shanxi Province from January 2020 to December 2023 were selected as the pregnancy group and divided into the early pregnancy group(<12 weeks),The second trimester group(12~28 weeks)and the third trimester group(>28 weeks).In addition,92 women who underwent physical examination in the hospital during the same period were selected as the control group,and the pregnancy group and the control group were divided into four groups:≤30 years old,>30~35 years old,>35~40 years old,and>40 years old respectively.The level of D-D was detected by latex immunoturbidimetry,and the level of D-D in pregnancy group and control group was compared.The D-D levels of pregnant women in different pregnancy periods were compared,and the correlation analysis was carried out.The difference of D-D level in different age groups of pregnancy group was analyzed.The D-D level P95 of pregnant women in different pregnancy periods was calculated,and the D-D reference interval of pregnant women in different pregnancy periods in our laboratory was established.Results The median D-D levels in the pregnancy group and the control group were 1.27(0.52,1.70)ug/ml and 0.36(0.23,0.54)µg/ml respectively,and the difference was statistically significant(U=31 671,P<0.05).The levels of D-D in early,middle and late pregnancy were 0.43(0.32,0.52)ug/ml,1.27(1.15,1.46)ug/ml and 1.82(1.68,1.89)μg/ml,respectively.The difference was statistically significant(H=35 8.93,P<0.05),and the level of D-D was positively correlated with pregnancy(r=0.925,P<0.05).The D-D levels of pregnant women in different age groups:≤30 years:1.25(0.47,1.68)μg/ml,>30~35 years:1.27(0.56,1.68)μg/ml,>35~40 years:1.36(0.48,1.83)μg/ml and>40 years:1.53(0.31,1.91)μg/ml,the difference was not statistically significant(H=0.71,P>0.05);The unilateral reference interval of D-D in pregnant women in our laboratory was<0.69 μg/ml in early pregnancy,<1.68 μg/ml in middle pregnancy and<2.07 μg/ml in late pregnancy.Conclusion The reference interval of D-D in pregnant women of different pregnancy periods in our laboratory was preliminarily established.It is helpful to more accurately guide the identification of VTE during pregnancy and the reasonable intervention of anticoagulant drugs.
