Pharmacodynamics and safety of ropivacaine used in epidural anesthesia for patients with abnor-mal liver function
Objective To analyze the pharmacodynamics of ropivacaine in patients with abnormal liver function during epidural block combined with general anesthesia,and to analyze the safety of ropivacaine.Methods By retrospective analysis,90 patients who underwent upper abdominal surgery under epidural block combined with general anesthesia in Marine Police Corps Hospital of the Chinese People's Armed Police Force from August 2021 to July 2023 were selected as the research object,and were divided into abnormal liver function group(n=38)and normal liver function group(n=52)according to whether they had abnormal liver function,and the sensory and motor block effects of the two groups after epidural injection of ropivacaine were collected.The hemodynamic indexes(heart rate,mean arterial pressure)of the two groups were collected before injection,10 min,20 min,60 min,90 min,120 min and 180 min,and the incidence of perioperative adverse reactions between the two groups was counted and compared.Results There was no significant difference in sensory and motor block effect between the two groups(P>0.05).There was no significant difference in modified Bromage score between groups(P>0.05).When the drug was injected for 120 min and 180 min,the average arterial pressure and heart rate of patients with abnormal liver function were lower than those without abnormal liver function(P<0.05).The incidence of hypotension and bradycardia in patients with abnormal liver function was significantly higher than that in patients with normal liver function(x2=24.354,4.409,P<0.05).Conclusion There is no difference be-tween patients with abnormal liver function and those without abnormal liver function in the effect of epidural block,but the perioperative heart rate and average arterial pressure of patients with abnormal liver function are not stable,and the incidence of hypotension and bradycardia is obviously increased.It is suggested that the principle of using ropivacaine several times should be followed,and the perioperative vital signs monitoring should be strengthened.
