Construction and application of pharmaceutical service model for chronic disease patients based on WeChat mini program
Objective To establish a pharmacy service model related to the home medication of patients with a WeChat mini-program,and improve the rationality,safety and timeliness of medication for chronic diseases patients.Methods Focusing on the pharmaceutical service needs of patients with chronic diseases,we integrated hospital pharmacy resources and constructd a pharmaceutical service model of"consultation and communication-popularization of medication knowledge-targeted online solutions to difficult problems"guided by the IMB(Information-Motivation-Behavioral Skills Model)model.It resolved many problems for chronic patients at home,including the complexity of medication varieties,improper timing of medication,failure to promptly identify adverse drug reactions,and the lack of symptomatic treatment.Results The pharmaceutical service model consists of three parts,namely,"WeChat group","WeChat official account"and"WeChat mini program".It has been put into practice since the end of December 2023.As of August 2024,more than 200 times of drug consultation,treatment plan restructuring and adverse reaction screening have been completed.After the intervention,the knowledge mastery rate of common medications for chronic diseases in the observation group increased from 31.6%before the intervention to 78.2%,and the correct rate of medication behavior increased from 22.0%before the intervention to 66.2%.The improvement effect after the intervention was better than that of the control group(P<0.05).At the same time,the number of patients'residences(cities/counties/districts)increased from 7 to 12,and the scope of hospital pharmaceutical services was significantly improved.Conclusion This model has low investment costs,is easy to operate,and has a wide range of audiences.Patients can enjoy timely and accurate pharmaceutical services without leaving their homes,ensuring the rationality and safety of medication for patients with chronic diseases at home.
