Diagnostic Value of CT Measurement of Bronchial Wall Thickness in Elderly Acute Stage of COPD
Objective:To assessment the diagnostic value of computer tomography(CT)measurement the bronchial wall thickness of elderly acute chronic obstructive pulmonary disease(COPD).Method:Data of 40 elderly patients with acute stage of COPD admitted to Jiujiang NO.1 People's Hospital from May 2021 to May 2022 were retrospectively selected as the observation group,and the data of 40 elderly people with normal cardiopulmonary function during the same period were included in the control group.CT quantitative measurement and pulmonary function test were given in the two groups.The airway wall thickness(WT),WT to airway diameter ratio(TDR)and airway wall area(WA)of the bronchial apical segment of the upper lobe of right lung and its sub-apical segment were compared between the two groups.The correlation between the above indicators and forced expiratory volume in the first second(FEV1)in patients with different severity of the observation group was analyzed.Result:The bronchial WT and TDR of the apical and sub-apical segment of the upper lobe of right lung and bronchial WA of in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).Different severity of COPD bronchial WT,TDR,WA in the observation group were compared,the bronchial WT of apical segment and sub-apical segment of the upper lobe of right lung,the bronchial TDR of apical segment of the upper lobe of right lung and the bronchial WA of sub-apical segment of the upper lobe of right lung in the serious group were higher than those of mild group and moderate group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the bronchial WT of apical segment of the upper lobe of right lung was negatively correlated with FEV1,the bronchial WT,TDR,WA of sub-apical segment of the upper lobe of right lung were negatively correlated with FEV1(P<0.05).Conclusion:The elderly patients with acute stage of COPD use CT quantitative measurement can reflect the bronchial wall thickness,help to judge the bronchial condition and assess the severity of the disease,and is closely related to pulmonary function.It can provide a reliable basis for the diagnosis and evaluation of COPD.
