Comparison on the Curative Effect of Two Surgical Methods on Hepatocellular Carcinoma and Their Influences on Immune Stress
Objective:To compare the curative effect of laparoscopic Glisson pedicle transection hepatectomy and laparotomy on hepatocellular carcinoma and their influences on immune stress.Method:The clinical data of 75 patients with hepatocellular carcinoma admitted to Longyan First Hospital Affiliated to Fujian Medical University from March 2019 to March 2022 were retrospectively analyzed.They were divided into group A and group B according to different surgical methods,40 cases in group A received laparoscopic Glisson pedicle transection hepatectomy and 35 cases in group B received laparotomy.The perioperative clinical indexes,changes in levels of liver function indexes[alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL)],inflammatory stress indexes[C reactive protein(CRP),procalcitonin(PCT),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α)]and immune T cells(CD3+,CD4+,CD8+)before and after surgery,and the occurrence of postoperative complications in the two groups were compared.Result:The operation time in group A was longer than that in group B,the operative blood loss was less than that of group B,and the first time of getting out of bed and the length of hospital stay were shorter than those of group B(P<0.05).After surgery,the levels of ALT,AST and TBIL in group A were lower than those in group B(P<0.05).After surgery,the levels of CRP,PCT,IL-8 and TNF-α in group A were lower than those in group B(P<0.05).After surgery,changes of CD3+,CD4+ and CD8+ in group A were better than those in group B(P<0.05).The incidence of postoperative complications in group A was lower than that in group B(P<0.05).Conclusion:Compared with laparotomy,laparoscopic Glisson pedicle transection hepatectomy has better curative effect,fewer effects on liver function and immune stress,better postoperative recovery and lower incidence of complications in patients with hepatocellular carcinoma.
