序贯血液净化疗法对有机磷中毒患者血清炎症因子及心肌损伤的影响

目的:分析序贯血液净化疗法在有机磷中毒患者中的治疗效果.方法:选取 2021 年 5 月—2022 年 5 月泉州市第一医院收治的 74 例有机磷中毒患者,按随机数字表法分为对照组和观察组,各37 例.对照组给予常规急诊+血液灌流治疗,观察组在对照组基础上加以序贯血液净化疗法.对比两组临床疗效、恢复情况、血清炎症因子、心肌损伤标志物.结果:观察组总有效率为 91.89%(34/37),高于对照组的 72.97%(27/37),胆碱酯酶(CHE)活性恢复时间为(90.47±6.09)h、昏迷时间为(75.29±5.31)h、住院时间为(12.33±1.26)d,均短于对照组的(110.21±8.36)h、(106.39±6.42)h、(15.49±1.89)d,差异均有统计学意义(P<0.05).治疗前,两组白介素-6(IL-6)、C反应蛋白(CRP)、白细胞计数(WBC)、降钙素原(PCT)、肌钙蛋白T(cTnT)、肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)相比,差异均无统计学意义(P>0.05);治疗后,观察组IL-6 为(75.39±6.31)ng/L、CRP为(6.37±1.02)mg/L、WBC 为(8.72±1.69)×109/L、PCT 为(1.14±0.38)μg/L、cTnT 为(0.41±0.05)μg/L、CK-MB为(6.20±1.07)ng/mL、LDH为(201.95±16.45)U/L,均低于对照组的(102.63±8.59)ng/L、(8.63±1.56)mg/L、(10.21±2.03)×109/L、(2.09±0.41)μg/L、(0.72±0.20)μg/L、(9.48±1.52)ng/mL、(254.37±20.63)U/L,差异均有统计学意义(P<0.05).结论:序贯血液净化疗法治疗有机磷中毒患者效果显著,可有效降低机体炎症因子水平,减轻心肌损伤.
Effects of Sequential Blood Purification Therapy on Serum Inflammatory Factors and Myocardial Damage in Patients with Organophosphorus Poisoning
Objective:To analyze the therapeutic effect of sequential blood purification therapy in patients with organophosphorus poisoning.Method:A total of 74 patients with organophosphorus poisoning admitted to Quanzhou First Hospital from May 2021 to May 2022 were selected and divided into the control group and the observation group according to the random number table method,with 37 cases in each group.The control group was treated with routine emergency + hemoperfusion therapy,while the observation group was treated with sequential blood purification therapy on the basis of the control group.The clinical efficacy,recovery,serum inflammatory factors and myocardial damage markers were compared between the two groups.Result:The total effective rate in the observation group was 91.89%(34/37),which was higher than 72.97%(27/37)in the control group,the recovery time of cholinesterase(CHE)activity was(90.47±6.09)h,coma time was(75.29±5.31)h and hospitalization time was(12.33±1.26)d,which were significantly shorter than(110.21±8.36)h,(106.39±6.42)h,and(15.49±1.89)d in the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in interleukin-6(IL-6),C reactive protein(CRP),white blood cell count(WBC),procalcitonin(PCT),cardiac troponin T(cTnT),creatine kinase isoenzyme(CK-MB)and lactate dehydrogenase(LDH)between the two groups(P>0.05);after treatment,in the observation group,the IL-6 was(75.39±6.31)ng/L,CRP was(6.37±1.02)mg/L,WBC was(8.72±1.69)×109/L,PCT was(1.14±0.38)μg/L,cTnT was(0.41±0.05)μg/L,CK-MB was(6.20±1.07)ng/mL and LDH was(201.95±16.45)U/L,which was lower than(102.63±8.59)ng/L,(8.63±1.56)mg/L,(10.21±2.03)×109/L,(2.09±0.41)μg/L,(0.72±0.20)μg/L,(9.48±1.52)ng/mL,(254.37±20.63)U/L in the control group,the differences were statistically significant(P<0.05).Conclusion:Sequential blood purification therapy has a significant effect in the treatment of patients with organophosphorus poisoning,which can effectively reduce the level of inflammatory factors in the body and reduce myocardial damage.
