Diagnostic Value of 25-hydroxyvitamin D3,Cystatin C and Interleukin-6 in Early Diabetic Nephropathy
Objective:To investigate the value of serum 25-hydroxyvitamin D3[25-(OH)D3],cystatin C(Cys C)and interleukin-6(IL-6)in the diagnosis of early diabetic nephropathy(DN).Method:A total of 90 patients with diabetes mellitus admitted to Shangrao Municipal Hospital from January 2022 to January 2023 were selected as diabetes mellitus group,and the patients were divided into normal albuminuria group(n=30),early nephrosis group(n=30)and clinical nephrosis group(n=30)according to their disease condition.And 30 healthy volunteers who received health examination at the same period were selected as the control group.Venous blood of all patients were taken to detect the levels of 25-(OH)D3,Cys C and IL-6.The differences of 25-(OH)D3,Cys C and IL-6 between the diabetes mellitus group and the control group were compared.The differences of 25-(OH)D3,Cys C and IL-6 in different stages of diabetes mellitus were analyzed.The value of single and combined detection of 25-(OH)D3,Cys C and IL-6 in the diagnosis of early DN were analyzed.Result:The 25-(OH)D3 of diabetes mellitus group was lower than that of control group,and Cys C and IL-6 of diabetes mellitus group were higher than those of control group,the differences were statistically significant(P<0.05).The 25-(OH)D3 of normal albuminuria group was higher than that of early nephrosis group and clinical nephrosis group,and the Cys C and IL-6 of normal albuminuria group were lower than those of early nephrosis group and clinical nephrosis group,the differences were statistically significant(P<0.05).The 25-(OH)D3 of early nephropathy group was higher than that of clinical nephropathy group,and Cys C and IL-6 were lower than those of clinical nephropathy group,the differences were statistically significant(P<0.05).The sensitivity and specificity of 25-(OH)D3 in the diagnosis of early DN respectively was 75.00%(45/60)and 81.67%(49/60).The sensitivity and specificity of Cys C in the diagnosis of early DN respectively was 83.33%(50/60)and 85.00%(51/60).The sensitivity and specificity of IL-6 in early diagnosis of DN respectively was 85.00%(51/60)and 61.67%(37/60).The sensitivity and specificity of the combined diagnosis of early DN respectively was 96.67%(58/60)and 93.33%(56/60).Conclusion:The detection of 25-(OH)D3,Cys C and IL-6 is high value in the early diagnosis of DN,and can evaluate the renal function damage of patients,which is helpful for patients to receive early intervention,control the progression of the disease and obtain relatively better clinical outcomes.
