目的:探讨孕早期孕妇中性粒细胞/淋巴细胞比值(NLR)、红细胞体积分布宽度(RDW)和血小板/淋巴细胞比值(PLR)水平与妊娠期糖尿病发生的相关性.方法:选取 2020 年 5 月—2021 年10 月于赣州市妇幼保健院进行孕检的 266 例妊娠 8~12 周的孕早期孕妇为研究对象,于妊娠 24~28 周时行75 g OGTT检查,根据检查结果将研究对象分为研究组(妊娠期糖尿病孕妇88例)和对照组(健康孕妇178例).于妊娠8~12周孕检时经XN2000全自动血细胞分析仪检测孕早期孕妇NLR、RDW和PLR水平;于妊娠24~28周孕检时经AU5821全自动生化仪检测孕妇空腹血糖(FPG)、服糖后1 h血糖(1 h PG)、服糖后2 h血糖(2 h PG)水平;Pearson分析孕妇孕早期NLR、RDW和PLR水平与FPG、1 h PG、2 h PG水平的相关性;受试者操作特征(ROC)曲线分析孕妇孕早期NLR、RDW和PLR水平对妊娠期糖尿病的诊断效能.结果:与对照组相比,研究组孕妇NLR、RDW、PLR、FPG、1 h PG、2 h PG水平均显著高(P<0.05);孕早期孕妇NLR、RDW和PLR水平与FPG、1 h PG、2 h PG水平呈正相关(P<0.05);NLR、RDW、PLR预测妊娠期糖尿病的AUC分别为 0.846、0.870、0.848,最佳截断值分别为 3.64、11.60%、132.23.结论:孕早期孕妇NLR、RDW和PLR水平显著高,与FPG、1 h PG、2 h PG水平均呈正相关,并且与妊娠期糖尿病的发生密切相关,对其具有较好的预测诊断效能.
Correlation between NLR,RDW and PLR in the Early Pregnancy and the Risk of Diabetes in Pregnancy
Objective:To investigate the relationship between the levels of neutrophil/lymphocyte ratio(NLR),red blood volume distribution width(RDW)and platelet/lymphocyte ratio(PLR)in early pregnancy and the occurrence of diabetes in pregnancy.Method:A total of 266 pregnant women in early pregnancy at 8-12 weeks of gestation who underwent pregnancy tests at Ganzhou Maternal and Child Health Hospital from May 2020 to October 2021 were selected for the study.75 g OGTT was performed at 24-28 weeks of pregnancy.According to the results,the study subjects were divided into the study group(88 pregnant women with gestational diabetes)and the control group(178 healthy pregnant women).The levels of NLR,RDW and PLR of pregnant women in the first trimester were detected by XN2000 automatic blood cell analyzer at 8-12 weeks of pregnancy;fasting plasma glucos(FPG),1 hour postprandial plasma glucose(1 h PG)and 2 hours postprandial plasma glucose(2 h PG)levels of pregnant women were detected by AU5821 automatic biochemical analyzer at 24-28 weeks of pregnancy.Pearson analyzed the correlation between the levels of NLR,RDW,PLR and FPG,1 h PG,2 h PG in the first trimester of pregnancy;receiver operator characteristic(ROC)curve was used to analyze the diagnostic efficacy of NLR,RDW and PLR levels in early pregnancy for diabetes in pregnancy.Result:Compared with the control group,the levels of NLR,RDW,PLR,FPG,1 h PG and 2 h PG in the study group were significantly higher(P<0.05).The levels of NLR,RDW and PLR in the first trimester of pregnancy were positively correlated with the levels of FPG,1 h PG and 2 h PG(P<0.05).The AUC predicted by NLR,RDW,and PLR for gestational diabetes was 0.846,0.870,and 0.848,respectively,and the optimal cutoff values were 3.64,11.60%,and 132.23,respectively.Conclusion:The levels of NLR,RDW and PLR in pregnant women in the early pregnancy are significantly increased,which is positively correlated with the levels of FPG,1 h PG and 2 h PG,and is closely related to the occurrence of diabetes in pregnancy,which has a good predictive diagnostic efficacy.
