摘要
目的:探讨高压氧(HBO)辅助治疗中老年 2 型糖尿病突发性聋的临床效果.方法:选取2022 年 8 月—2023 年 7 月在保定市第一中心医院全科医疗科及耳鼻咽喉科就诊的 60 例中老年 2 型糖尿病突发性聋患者为研究对象,通过随机数字表法分为试验组和对照组,每组 30 例.对照组接受糖皮质激素、扩血管改善循环及营养神经等治疗,试验组在对照组的基础上给予HBO辅助治疗.治疗 1 个疗程后,比较两组患者的纯音听阈、血糖、中性粒细胞淋巴细胞比值(NLR)、血小板淋巴细胞比值(PLR)、临床疗效、不良反应及并发症发生的情况.结果:经治疗后两组患者的纯音听阈、血糖、NLR、PLR指标均显著下降,且试验组纯音听阈、血糖、NLR、PLR均低于对照组,差异均有统计学意义(P<0.05);试验组治疗后听阈增益高于对照组,差异有统计学意义(P<0.05);试验组综合疗效(86.67%)显著高于对照组(60.00%),差异有统计学意义(P<0.05).两组患者不良反应发生率及并发症发生率比较差异均无统计学意义(P>0.05).结论:高压氧辅助治疗针对中老年2型糖尿病突发性聋患者,可明显改善患者的血糖与炎性指标,显著提高临床效果,且安全性可.
Abstract
Objective:To investigate the clinical effect of hyperbaric oxygen-assisted treatment of sudden deafness in middle-aged and elderly patients with type 2 diabetes.Method:A total of 60 middle-aged and elderly patients with sudden deafness with type 2 diabetes treated in the General Medicine Department and Otolaryngology Department of Baoding First Central Hospital from August 2022 to July 2023 were included in this study,they were divided into experimental group(30 cases)and control group(30 cases)according to the random number table method.The control group received glucocorticoid,vascular dilation and nerve nutrition therapies.The experimental group was given HBO therapy on the basis of the control group.After one course of treatment,the pure tone hearing threshold,blood glucose,neutrophil lymphocyte ratio(NLR),platelet lymphocyte ratio(PLR),clinical efficacy,incidences of adverse reactions and complications between the two groups of patients were compared.Result:After treatment,the pure tone hearing thresholds,blood glucose,NLR and PLR in the two groups were lower than those before treatment,and pure tone hearing threshold,blood glucose,NLR and PLR in experimental group were lower than those in control group,the differences were statistically significant(P<0.05).After treatment,the hearing threshold gain of the experimental group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).The total effective rate of the experimental group(86.67%)was significantly higher than that of the control group(60.00%),the difference was statistically significant(P<0.05).There were no significant differences in the incidences of adverse reactions and complications between the two groups(P>0.05).Conclusion:Hyperbaric oxygen-assisted treatment of sudden deafness in middle-aged and elderly patients with type 2 diabetes can reduce the levels of blood glucose and inflammatory factors,improve the clinical therapeutic effect and safety.
