Analysis of the Therapeutic Effect and Prognostic Related Influencing Factors of Laparoscopic Radical Resection of Colorectal Cancer
Objective:To analyze the therapeutic effect and prognostic influencing factors of laparoscopic radical of resection colorectal cancer in patients with colorectal cancer.Method:A total of 90 patients with colorectal cancer treated in Beihai Hospital of Yantai from February 2019 to February 2021 were selected.All patients underwent laparoscopic radical resection of colorectal cancer,and the therapeutic effect was observed.Patients were followed up for 2 years after surgery and divided into good prognosis group and poor prognosis group according to their clinical outcome(survival or death).The age,gender and other data of patients were collected,and the prognostic influencing factors were statistically analyzed by multivariate logistic regression analysis model.Result:Among the 90 patients,a total of 50 cases(55.56%)had complete response(CR),34 cases(37.78%)had partial response(PR),5 cases(5.56%)had stable disease(SD),and 1 case(1.11%)had progressive disease(PD),the total effective rate was 93.33%(84/90).After 2 years of follow-up,69 of the 90 patients survived,and the survival rate was 76.67%(69/90).Univariate analysis showed that there were no significant differences in the proportions of age,gender,body mass index(BMI),education level,smoking history,hypertension history,tumor diameter and tumor type between the poor prognosis group and the good prognosis group(P>0.05).The proportions of poorly differentiation,clinical stage Ⅲ-Ⅳ,vascular invasion and nerve invasion of poor prognosis group were higher than those of good prognosis group,the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that poorly differentiation[β=2.059,OR=7.840,95%CI(2.530,24.291)],clinical stage Ⅲ-Ⅳ[β=2.485,OR=12.000,95%CI(3.203,44.956)],vascular invasion[β=1.828,OR=6.223,95%CI(1.892,20.466)],nerve invasion[β=2.285,OR=9.821,95%CI(3.224,29.916)]were the influencing factors of poor prognosis of patients after laparoscopic radical of resection colorectal cancer(P<0.05).Conclusion:Laparoscopic radical of resection colorectal cancer has a significant effect on patients with colorectal cancer,and many factors affect the prognosis of patients,such as poorly differentiation,clinical stage Ⅲ-Ⅳ,vascular invasion,nerve invasion and so on.
