Effect of Direct Anterior Approach on Gait Recovery and Lower Limb Venous Insufficiency after Primary Unilateral Total Hip Arthroplasty
Objective:To investigate and analyze the effect of direct anterior approach on gait recovery and lower limb venous insufficiency after primary unilateral total hip arthroplasty.Method:A total of 112 patients with primary unilateral femoral neck fracture diagnosed and treated in Fuxin Mine General Hospital of Liaoning Health Industry Group from January 2021 to November 2022 were selected as the study objects,and 112 patients were divided into study group(n=56)and control group(n=56)according to random number table method.Both groups were treated with total hip arthroplasty,the study group with direct anterior approach and the control group with posterolateral approach.The perioperative period indexes,hip joint function,gait and lower limb venous insufficiency situation were observed and recorded.Result:The operation time and postoperative hospitalization time of the study group were shorter than those of the control group,and the postoperative drainage volume and intraoperative bleeding volume of the study group were lower than those of the control group,the differences were statistically significant(P<0.05).The excellent and good rate of hip joint function in the study group was lower than that in the control group(82.14%vs 64.29%),the difference was statistically significant(P<0.05).The incidence of lower limb venous insufficiency in the study group was 0,compared with 10.71%in the control group,the difference was statistically significant(P<0.05).3 days after surgery,there were no significant differences in step length and step speed between the two groups(P>0.05).3 months after surgery,the step length of the study group was longer than that of the control group,and the step speed of the study group was faster than that of the control group,the difference were statistically significant(P<0.05).Conclusion:Direct anterior approach primary unilateral total hip arthroplasty can shorten the operation time,reduce the intraoperative bleeding volume and the incidence of lower limb venous insufficiency,and is beneficial to the recovery of hip joint function.
