Prediction Value of Caprini Score Combined with Fibrinogen Level for Deep Vein Thrombosis of Lower Extremity in Patients with Lower Extremity Fracture
Objective:To explore the prediction value of Caprini score combined with fibrinogen(FIB)level for deep vein thrombosis(DVT)of lower extremity in patients with lower extremity fracture.Method:A total of 100 patients with lower extremity fracture admitted to the 904th Hospital of the Joint Logistic Support Force of the Chinese People's Liberation Army from January 1st,2020 to September 30th,2022 were selected as the research objects.The research objects were divided into DVT group(n=11)and non-DVT group(n=89)according to the results of preoperative color Doppler ultrasonography of deep vein of lower limb.FIB level and Caprini score were compared between DVT group and non-DVT group.Based on the model formula:Y=β0+β1×A+β2×B,the Caprini score and FIB regression model was constructed.Receiver operating characteristic(ROC)curve was established to analyze the application value of Caprini score,FIB,Caprini score combined with FIB in predicting DVT of lower extremity in patients with lower extremity fracture.Result:The Caprini score and FIB level in DVT group were higher than those in non-DVT group(P<0.05).The binary logistic regression model of Caprini score and FIB was:Y=-21.556+2.637×FIB+1.997×Caprini score.According to ROC curve analysis,the area under curve(AUC)of DVT of lower extremity predicted by FIB was 0.908,the sensitivity was 81.82%and the specificity was 92.13%.The AUC of DVT of lower extremity predicted by Caprini score was 0.939,the sensitivity was 90.91%and the specificity was 85.39%.The AUC of DVT of lower extremity predicted by Caprini score combined with FIB was 0.972,the sensitivity was 100%and the specificity was 86.52%.Conclusion:Patients with lower extremity fracture with DVT of lower extremity have higher Caprini scores before surgery,and FIB levels show an abnormal increase,and Caprini scores combined with FIB have a higher value in predicting DVT of lower extremity.
