Comparison of the Application Effect of Percutaneous Nephrolithotomy and Flexible Ureteroscope Holmium Laser Lithotripsy in Renal and Ureteral Calculi
Objective:To compare the effect of percutaneous nephrolithotomy(PCNL)and flexible ureteroscope holmium laser lithotripsy(FURL)in the treatment of renal and ureteral calculi.Method:A total of 60 patients with kidney and ureteral calculi admitted to Dexing People's Hospital from March 2019 to February 2022 were selected as the study objects and randomly divided into control group and observation group,with 30 cases in each group.The control group received PCNL treatment,and the observation group received FURL treatment.Both groups were followed up for 1 months after surgery.Operation related indexes,stress response,renal function,quality of life and complications were compared between the two groups.Result:There were no significant differences in stone clearance rate and operation time between the two groups(P>0.05).The intraoperative bleeding volume in the observation group was(20.46±2.17)mL,less than(43.59±3.48)mL in the control group,the hospital stay in the observation group was(3.84±0.52)d,shorter than(5.72±1.03)d in the control group,the incidence of complications in the observation group was 6.67%(2/30),lower than 30.00%(9/30)in the control group,the differences were statistically significant(P<0.05).Before surgery,there were no significant differences in the levels of cortisol(Cor),adrenocorticotropic hormone(ACTH),norepinephrine(NE),epinephrine(E),serum creatinine(Scr),blood urea nitrogen(BUN)and scores of each dimension of 36-item short form health survey(SF-36)between the two groups(P>0.05).After surgery,the Cor,ACTH,NE,E,Scr and BUN in the observation group respectively was(235.97±14.21)nmol/L,(36.97±3.84)ng/L,(756.75±38.73)pmol/L,(483.42±25.59)pmol/L,(97.69±10.54)μmol/L,(6.53±1.81)mmol/L,were lower than(341.86±18.75)nmol/L,(48.82±4.15)ng/L,(839.84±40.76)pmol/L,(571.69±28.96)pmol/L,(108.72±12.76)μmol/L,(7.89±2.43)mmol/L in the control group,scores of each dimension of SF-36 respectively was(79.87±4.12),(80.23±4.08),(79.64±3.76),(78.63±3.98),(78.73±3.41),(79.64±3.97),(80.36±4.01),(82.63±4.28)scores,were higher than(71.35±3.46),(74.84±3.59),(70.75±3.18),(72.57±3.49),(72.49±2.95),(71.85±3.28),(73.59±3.22),(76.59±3.47)scores in the control group,the differences were statistically significant(P<0.05).Conclusion:PCNL and FURL in the treatment of patients with renal and ureteral calculi can achieve excellent stone clearance effects,but FURL has less trauma to the body,less stress response,less damage to patients'kidney function,and helps patients to recover quickly after surgery,thus improving their quality of life,and has fewer complications.
