纤维支气管镜吸痰联合肺泡灌洗在重症肺炎并发呼吸衰竭中的应用效果观察

摘要
目的:浅析纤维支气管镜吸痰联合肺泡灌洗对重症肺炎并发呼吸衰竭的疗效.方法:2022年1-12 月,将鄂钢医院收治的 60 例重症肺炎并发呼吸衰竭患者依照随机数字表法分为观察组(接受纤维支气管镜吸痰联合肺泡灌洗干预)和对照组(接受传统吸痰干预),各 30 例.比较两组临床肺部感染评分(CPIS)、动脉血气、机械通气时间、吸痰次数、疗效和操作安全性.结果:两组干预前的CPIS评分和动脉血气指标比较,差异均无统计学意义(P>0.05);干预后,观察组的CPIS评分低于对照组,动脉血氧分压与动脉血氧饱和度均高于对照组,差异均有统计学意义(P<0.05).观察组机械通气时间短于对照组,吸痰次数少于对照组,治疗总有效率高于对照组,差异均有统计学意义(P<0.05).全体患者治疗期间均无安全事故发生.结论:重症肺炎并发呼吸衰竭患者接受纤维支气管镜吸痰联合肺泡灌洗干预,可取得良好效果.
Abstract
Objective:To simple analyze the curative effect of fiber bronchoscope sputum aspiration combined with alveolar lavage on severe pneumonia complicated with respiratory failure.Method:From January to December 2022,60 patients with severe pneumonia complicated with respiratory failure admitted to Egang Hospital were divided into observation group(received fiber bronchoscope sputum aspiration combined with alveolar lavage intervention)and control group(received traditional sputum aspiration intervention)according to random number table method,with 30 cases in each group.Clinical pulmonary infection score(CPIS),arterial blood gas,mechanical ventilation time,sputum aspiration times,efficacy and operation safety were compared between the two groups.Result:There were no significant differences in CPIS scores and arterial blood gas indexes between the two groups before intervention(P>0.05).After intervention,the CPIS scores of the observation group was lower than that of the control group,and the arterial partial pressure of oxygen and arterial oxygen saturation were higher than those of the control group,the differences were statistically significant(P<0.05).The mechanical ventilation time of observation group was shorter than that of control group,the sputum aspiration times was less than that of control group,and the total effective rate of treatment was higher than that of control group,the differences were statistically significant(P<0.05).No safety accident occurred in all patients during treatment.Conclusion:Patients with severe pneumonia complicated with respiratory failure can get good good effect when receiving the intervention of fiber bronchoscope sputum aspiration combined with alveolar lavage.
