摘要
目的:探讨Henry入路三角肌双间隙治疗AOC型桡骨远端骨折的临床效果.方法:选取2021 年 6 月—2023 年 4 月高安市人民医院收治的AOC型桡骨远端骨折患者 102 例作为研究对象,按随机数字表法分为两组,各 51 例,对照组采用传统入路方式治疗,观察组采用Henry入路三角肌双间隙治疗.观察两组手术相关指标、疼痛程度[视觉模拟评分法(VAS)]、手部功能恢复情况[上肢功能障碍评定量表(DASH)]、术后并发症发生情况.结果:观察组手术时间、骨折愈合时间及术后住院时间均短于对照组(P<0.05);观察组术中出血量少于对照组(P<0.05).两组术后 1 d、1 个月、3 个月的VAS评分与术前相比,差异均有统计学意义(P<0.05);观察组术后 1 d、1 个月、3 个月的VAS评分均低于对照组(P<0.05).术后 3 个月,观察组DASH评分低于对照组(P<0.05).观察组并发症发生率为 3.92%,对照组为 15.69%,差异有统计学意义(P<0.05).结论:Henry入路三角肌双间隙治疗AOC型桡骨远端骨折可减少术中出血量,缩短手术时间及术后住院时间,有效促进骨折愈合与手部功能恢复,减轻疼痛且并发症少.
Abstract
Objective:To investigate the clinical effect of Henry approach deltoid muscle double space in the treatment of AOC type distal radius fracture.Method:A total of 102 patients with AOC type distal radius fracture admitted to Gao'an People's Hospital from June 2021 to April 2023 were selected as the study objects and divided into two groups according to the random number table method,with 51 cases in each group.The control group was treated with the traditional approach,and the observation group was treated with Henry approach for deltoid muscle double space.Operation-related indexes,pain degree[visual analogue scale(VAS)],recovery of hand function[disabilities of the arm,shoulder and hand(DASH)]and incidence of postoperative complications of the two groups were observed.Result:The operation time,fracture healing time and postoperative hospital stay in the observation group were shorter than those in the control group(P<0.05);and the intraoperative blood loss in the observation group was less than that in the control group(P<0.05).The VAS scores of the two groups at 1 d,1 month and 3 months after surgery compared with those before surgery,the differences were statistically significant(P<0.05);VAS scores of observation group were lower than those of control group at 1 d,1 month and 3 months after surgery(P<0.05).Three months after surgery,the DASH score of observation group was lower than that of control group(P<0.05).The incidence of complications was 3.92%in the observation group and 15.69%in the control group,the difference was statistically significant(P<0.05).Conclusion:Henry approach deltoid muscle double space treatment for AOC distal radius fracture can reduce intraoperative blood loss,shorten operative time and postoperative hospital stay,effectively promote fracture healing and hand function recovery,relieve pain and reduce complications.
