Diagnostic Value of Cardiac Color Doppler Ultrasonography in Patients with Coronary Heart Disease and Heart Failure with Different Cardiac Function Grades
Objective:To explore the diagnostic value of of cardiac color Doppler ultrasonography in patients with coronary heart disease and heart failure with different cardiac function grades.Method:A total of 80 elderly patients with with coronary heart disease and heart failure who received treatment in Ganzhou Women and Children's Health Care Hospital from August 2021 to July 2023 were selected as the experimental group,and 80 healthy volunteers who received health examination in our hospital during the same period were selected as the control group.The subjects in the experimental group were divided into experimental group 1(grade Ⅱ,n=25),experimental group 2(grade Ⅲ,n=32)and experimental group 3(grade Ⅳ,n=23)according to the cardiac function grades of New York heart association(NYHA).The results of cardiac color Doppler ultrasonography[left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),ratio of early and late mitral diastolic blood flow peak velocity(E/A),longitudinal strain(LS),radial strain(RS),and circumferential strain(CS)]between the experimental group and the control group and patients with different cardiac function grades of NYHA were compared,and the correlation between the above parameters and cardiac function grades of NYHA were analyzed.Result:LVESD and LVEDD of experimental group were significantly bigger than those of control group,LVEF and RS were lower than those of control group,LS and CS were higher than those of control group,and the E/A was significantly smaller than that of the control group,the differences were statistically significant(P<0.05).LVESD and LVEDD of experimental group 1 were significantly smaller than those of experimental group 2 and experimental group 3,LVEF,RS were significantly higher than those of experimental group 2 and experimental group 3,LS and CS were lower than those of experimental group 2 and experimental group 3,and the E/A were smaller than those of experimental group 2 and experimental group 3,the differences were statistically significant(P<0.05).LVESD and LVEDD of experimental group 2 were significantly smaller than those in experimental group 3,LVEF,RS were significantly higher than those of experimental group 3,LS and CS were lower than those of experimental group 3,and the E/A was smaller than that of experimental group 3,the differences were statistically significant(P<0.05).Spearman results showed that LVESD,LVEDD,LS,CS were positively correlated with NYHA grade(rs=0.432,0.473,0.501,0.536,P<0.05).LVEF,E/A,RS were negatively correlated with NYHA grade(rs=-0.461,-0.531,-0.426,P<0.05).Conclusion:Cardiac color Doppler ultrasonography can determine the changes of left ventricular and myocardial strain parameters in patients with coronary heart disease and heart failure with different cardiac function grades,and can evaluate the severity of patients'disease.
