目的:分析急性脑梗死患者神经功能损伤程度与自然杀伤(NK)细胞、中性粒细胞与淋巴细胞比值(NLR)、同型半胱氨酸(Hcy)的关系.方法:回顾性选取 2021 年 1 月—2023 年 10 月宜春市第二人民医院收治的 80 例急性脑梗死患者的临床资料,将其归为观察组,根据美国国立卫生研究院卒中量表(NIHSS)对观察组患者不同神经功能损伤程度进行分组,分为轻度损伤组 40 例(NIHSS评分≤8 分)、重度损伤组 40 例(NIHSS评分>8 分),另选取同一时期 40 例正常健康体检者,将其归为对照组.比较观察组治疗前与对照组NK细胞、NLR、Hcy水平.进行单因素及多因素分析,探讨影响急性脑梗死患者神经功能损伤的主要影响因素,之后展开Spearman相关性分析,评估神经功能损伤程度与NK细胞、NLR、Hcy之间的关系.结果:观察组NK细胞水平低于对照组,差异有统计学意义(P<0.05);NLR、Hcy水平均高于对照组,差异均有统计学意义(P<0.05).经单因素及多因素分析,患者自发病至入院治疗时间>4.5 h、NK细胞、NLR、Hcy水平均为急性脑梗死患者神经功能损伤独立影响因素(P<0.05).NK细胞与神经功能损伤程度之间呈负相关(P<0.05);NLR、Hcy与神经功能损伤程度之间呈正相关(P<0.05).结论:NK细胞、NLR、Hcy水平均与急性脑梗死患者神经功能损伤程度相关,可作为临床评估参考指标.
Correlation Analysis of the Degree of Neurological Impairment,NK Cells,NLR and Hcy in Patients with Acute Cerebral Infarction
Objective:To analyze the correlation of the degree of neurological impairment,natural killer(NK)cells,neutrophil-to-lymphocyte ratio(NLR)and homocysteine(Hcy)in patients with acute cerebral infarction.Method:Clinical data of eighty patients with acute cerebral infarction who were admitted to the Second People's Hospital of Yichun City from January 2021 to October 2023 were selected retrospectively,and they were included in the observation group,according to the national institutes of health stroke scale(NIHSS)score,they were divided into mild impairment group(40 cases,NIHSS score≤8 point)and severe impairment group(40 cases,NIHSS score>8 points).Meanwhile,40 healthy individuals were selected as the control group.The levels of NK cells,NLR,and Hcy between the observation group before treatment and the control group were compared.Univariate analysis and multivariate analysis were conducted to identify the main influencing factors of neurological impairment in patients with acute cerebral infarction.Spearman correlation analysis was conducted to discuss the relationship among the degree of neurological impairment and NK cells,NLR and Hcy.Result:The NK cell level in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05);NLR and Hcy levels were higher than those in the control group,the differences were statistically significant(P<0.05).Univariate analysis and multivariate analysis found that the time from onset to admission for treatment in patients with acute cerebral infarction was>4.5 hours,NK cells,NLR and Hcy levels were independent influencing factors of neurological impairment in patients with acute cerebral infarction(P<0.05).The levels of NK cells was negatively correlated with the degree of neurological impairment(P<0.05);NLR and Hcy were positively correlated with the degree of neurological impairment(P<0.05).Conclusion:The levels of NK cells,NLR and Hcy are correlated with the degree of neurological impairment in patients with acute cerebral infarction.They can be used as reference indicators for clinical evaluation.
