Effect of GeneXpert MTB/RIF Combined with Peripheral Blood Cell Analysis for the Clinical Diagnosis of Pulmonary Tuberculosis
Objective:To observe the effect of Mycobacterium tuberculosis/Rifampicin resistance real-time fluorescence quantitative nucleic acid amplification detection(GeneXpert MTB/RIF)combined with peripheral blood cell analysis for the clinical diagnosis of suspected pulmonary tuberculosis(TB)with sputum smear negative.Method:A total of 66 sputum smear negative suspected TB patients admitted to Ji'an First People's Hospital from October 2022 to September 2023 were selected.All patients underwent GeneXpert MTB/RIF test and peripheral blood cell analysis for CD45+CD3+,CD4+CD45+and CD163+,and patients were identified by sputum culture and strain indentification for tuberculosis.The value of GeneXpert MTB/RIF test and peripheral blood cell analysis for diagnosing sputum smear negative suspected TB was analyzed.Result:Among 66 patients,36 patients with suspected pulmonary tuberculosis were diagnosed as TB patients(TB group)by sputum culture and strain identification,and 30 patients were non-TB patients(non-TB group).GeneXpert MTB/RIF was detected in 30 positive patients,with a detection rate of 83.33% .The CD45+CD3+of TB group was lower than that of non-TB group,CD4+CD45+and CD163+were higher than those of non-TB group,the differences were statistically significant(P<0.05).The results of receiver operating characteristic(ROC)curve analysis showed that the area under the curve(AUC)of the combination of CD4+CD45+,CD45+CD3+,CD163+and GeneXpert MTB/RIF detection was higher than that of single detection,the difference was statistically significant(P<0.05).The sensitivity of GeneXpert MTB/RIF combined with peripheral blood cell analysis(CD4+CD45+,CD45+CD3+and CD163+)was 92.87% .Conclusion:GeneXpert MTB/RIF detection and peripheral blood cell analysis combined with sputum smear negative suspected TB has significant value and high detection rate.
