The Value of Serological Test Combined with Cytological Test in Prenatal Diagnosis of Elderly Parturients
Objective:To explore the application value of serological test combined with cytological test in prenatal diagnosis of elderly parturients.Method:The clinical data of 120 elderly parturients admitted to the Ninth People's Hospital of Shenzhen from April 2022 to April 2023 were retrospectively analyzed.The levels of plasma protein-A(PAPP-A),β-human chorionic gonadotrophin(β-hCG),alpha-fetoprotein(AFP)and unconjugated estriol(uE3)in parturients were detected by fully automatic time-resolved fluoroimmunoassay instrument for serological test,and cytological test were performed by noninvasive DNA.The results of chromosome examination after amniocentesis as the gold standard,the detection efficiency of the two detection methods for fetal chromosome abnormality was evaluated by the receiver operating characteristic(ROC)curve,and the pregnancy outcome was followed up.Result:Among 120 parturients,32 cases of fetal chromosomal abnormality were found by chromosome karyotype analysis.The AUC of cytology and serology alone and in combination were 0.882,0.878 and 0.952,respectively,sensitivity was 84.4%,81.2%and 93.8%,respectively,and the specificity was 92.0%,94.3%and 96.6%,respectively.Of the 32 parturients in which fetal chromosomal abnormalities were detected,30 chose to parturition induction and 2 chose normal delivery,with the normal delivery of the fetus 47,XXY and 47,XYY.Conclusion:Serological test combined with cytological test is of great value in the prenatal diagnosis of elderly parturients,which can effectively improve the diagnostic accuracy and optimize clinical decision-making,and is of great significance for protecting the health of mother and child in elderly parturients.
