摘要
目的:研究双胎妊娠产妇不良妊娠结局的影响因素,以及宫颈管长度、血清白蛋白、体重增长情况对双胎妊娠产妇不良结局的预测价值.方法:选取 2021 年 3 月—2023 年 2 月瑞金市妇幼保健院接诊的 102 例双胎妊娠产妇,比较两组临床资料、宫颈管长度、血清白蛋白水平等,根据妊娠结局将其分为不良妊娠结局组(n=50)、正常组(n=52).采用logistic回归分析法分析双胎妊娠产妇不良妊娠结局的影响因素,绘制受试者操作特征(ROC)曲线分析宫颈管长度联合血清白蛋白、体重增长情况预测双胎妊娠产妇不良妊娠结局的效能.结果:102 例研究对象中共有 50 例出现不良妊娠结局,发生率为49.02%.两组年龄、体重增长、血清白蛋白、宫颈管长度比较,差异均有统计学意义(P<0.05).logistic回归分析结果显示,年龄是双胎妊娠产妇不良妊娠结局的危险因素,体重增长、血清白蛋白、宫颈管长度是双胎妊娠产妇不良妊娠结局的保护因素(P<0.05).ROC曲线显示,宫颈管长度联合血清白蛋白、体重增长情况预测双胎妊娠产妇不良妊娠结局的AUC为 0.916.结论:宫颈管长度、血清白蛋白、体重增长是双胎妊娠产妇不良妊娠结局的保护因素,三者联合检测预测双胎妊娠产妇不良妊娠结局的效能较高.
Abstract
Objective:To study the influencing factors of adverse pregnancy outcomes in puerpera with twin pregnancy,and the predictive value of cervical canal length,serum albumin and weight increase in the adverse pregnancy outcomes of puerpera with twin pregnancy.Method:A total of 102 puerpera with twin pregnancy were selected from Ruijin Maternal and Child Health Hospital from March 2021 to February 2023.The clinical data,cervical canal length and serum albumin level of the two groups were compared,and they were divided into adverse pregnancy outcomes group(n=50)and normal group(n=52)according to pregnancy outcomes.logistic regression analysis was used to analyze the influencing factors of adverse pregnancy outcomes in puerpera with twin pregnancy,and receiver operating characteristics(ROC)curve were drawn to analyze the efficacy of cervical canal length combined with serum albumin and weight increase in predicting adverse pregnancy outcomes in twin pregnancy.Result:A total of 50 of 102 subjects had adverse pregnancy outcomes,with an incidence of 49.02%.There were significant differences in age,weight increase,serum albumin and cervical canal length between the two groups(P<0.05).logistic regression analysis showed that age was a risk factor for adverse pregnancy outcomes in puerpera with twin pregnancy,while weight increase,serum albumin and cervical canal length were protective factors for adverse pregnancy outcomes in puerpera with twin pregnancy(P<0.05).ROC curve showed that the AUC of cervical canal length combined with serum albumin and weight increase increase in predicting adverse pregnancy outcomes of puerpera with twin pregnancy was 0.916.Conclusion:Cervical canal length,serum albumin and weight increase are protective factors for adverse pregnancy outcomes in puerpera with twin pregnancy,and the combined detection of the three factors is more effective in predicting adverse pregnancy outcomes in puerpera with twin pregnancy.
