Application of Three-dimensional Ultrasound Vitreous Imaging in Fetal Cardiac Screening during Pregnant Metaphase
Objective:To investigate the application value of three-dimensional ultrasound vitreous imaging in fetal cardiac screening during pregnant metaphase.Method:A total of 848 gravidas with lower-than-expected fetal growth rate of pregnant metaphase who underwent prenatal examination in Fuzhou Sixth Hospital from October 2021 to September 2023 were retrospectively selected as the study objects,and all of them underwent two-dimensional ultrasound and color Doppler ultrasound combined with three-dimensional ultrasound vitreous imaging.The diagnostic results of different types of fetal cardiac malformations in ultrasonic examination and their display in different ultrasonic sections were recorded,and the diagnosis of fetal heart malformations in different ultrasonic sections were recorded.Result:There were 81 cases fetal heart malformation by ultrasonography,and postnatal visits and parturition induction recorded 87 cases fetal heart malformation,with a diagnostic compliance rate of 93.10%,and the highest was ventricular septal defect(40.74%),followed by complete transposition of great arteries(25.93%),double outlet of right ventricle(20.99%),ventricle dysplasia(7.41%),pulmonary atresia(2.47%),single ventricle(1.23%),and persistent truncus arteriosus(1.23%).The single ventricle was mainly abnormal in the four-chamber view,the arterial valve and trunk malformations were mainly abnormal in the three-vessel view and the left and right ventricular outflow tract view,and the atrioventricular septal malformations were mainly abnormal in the left and right ventricular outflow tract view and the four-chamber view.The diagnostic rates of four-chamber incisal plane+left and right ventricular outflow tract section,four-chamber incisal plane+three vessel plane and four-chamber incisal plane+aortic root short axis section(74.71%,88.51%,58.62%)were higher than those of four-chamber incisal plane(43.68%),the differences were statistically significant(P<0.05).Using postnatal visits and parturition induction records as the gold standard,the positive predictive value and negative predictive value of 904 cases of fetal heart malformations were 93.10%and 99.39%respectively,and the diagnostic accuracy was 98.78%.Conclusion:Three-dimensional ultrasound vitreous imaging has high diagnostic accuracy and is easy to operate in fetal heart screening in pregnant metaphase of lower-than-expected fetal growth rate.
