Clinical Efficacy of Movable Platform Unicompartmental Knee Arthroplasty in the Treatment of Severe Osteoarthritis in the Medial Compartment of the Knee Joint
Objective:To investigate the clinical efficacy of movable platform unicompartmental knee arthroplasty(UKA)and total knee arthroplasty(TKA)in the treatment of severe osteoarthritis(OA)in the medial compartment of the knee joint.Method:A total of 50 patients with severe OA in the medial compartment of the knee joint who were admitted to Bengbu Hospital of Shanghai General Hospital from January 2022 to September 2023 were selected as the study subjects,and were divided into UKA group(n=25)and TKA group(n=25)by the random number table method.The operation time,incision length,intraoperative blood loss,hospital stay,time for knee joint flexion 90°,visual analogue scale(VAS)score,the Western Ontario and McMaster universities osteoarthritis index(WOMAC)score,hospital for special surgery(HSS)score,range of motion(ROM)and forgotten joint score(FJS),mechanical medial proximal tibial angle(mMPTA),hip-knee-ankle angle(HKA),posterior tibial slope(PTS)and femorotibial angle(FTA)were compared between the two groups.Result:All operations were successfully completed in both groups.Compared with the TKA group,incision was smaller,intraoperative blood loss was less,operation time,hospital stay and time for knee flexion 90° were shorter in the UKA group,the differences were statistically significant(P<0.05).At 6 months postoperatively,the VAS score,ROM and FJS in the UKA group were better than those in the TKA group,the differences were statistically significant(P<0.05);there were no significant differences in WOMAC and HSS scores between the two groups(P>0.05).At the preoperative and 6 months postoperatively,X-ray showed that there were no significant differences in mMPTA,HKA,PTS and FTA between the two groups(P>0.05).There were no surgery-related complications in both groups during the follow-up period.Conclusion:For severe OA in the medial compartment of the knee joint,compared with TKA,the movable platform UKA has less trauma,more obvious pain relief and better joint function recovery.
