目的:探讨骨科机器人辅助复位空心螺钉内固定治疗老年外展嵌插型股骨颈骨折的效果.方法:回顾性分析 2021 年 3 月—2023 年 11 月在南昌县人民医院接受诊治的 102 例老年外展嵌插型股骨颈骨折患者的临床资料.根据手术方式的不同分为两组,各 51 例;机器人组行骨科机器人辅助复位空心螺钉内固定治疗,传统组行传统复位空心螺钉内固定治疗.比较两组手术时间、术中透视次数、术中导针钻孔次数、内固定失效率及术后 6 个月的髋关节功能.结果:机器人组手术时间短于传统组,术中透视次数、术中导针钻孔次数均少于传统组,差异均有统计学意义(P<0.05).机器人组内固定失效率低于传统组,差异有统计学意义(P<0.05).术后 6 个月,机器人组Harris髋关节评分高于传统组,差异有统计学意义(P<0.05).结论:骨科机器人辅助复位空心螺钉内固定治疗老年外展嵌插型股骨颈骨折的效果良好,可缩短手术时间,减少术中透视次数和术中导针钻孔次数,更有利于髋关节功能恢复.
Application Effect of Orthopedic Robot Assisted Reduction with Hollow Screw Internal Fixation in the Treatment of Elderly Abduction and Insertion Femoral Neck Fracture
Objective:To investigate the effect of orthopedic robot assisted reduction with hollow screw internal fixation in the treatment of elderly abduction and insertion femoral neck fracture.Method:The clinical data of 102 elderly patients with abduction and insertion femoral neck fracture treated in Nanchang County People's Hospital from March 2021 to November 2023 were retrospectively analyzed.They were divided into two groups according to the different operation methods,with 51 cases in each group.Among them,the robot group underwent orthopedic robot assisted reduction with hollow screw internal fixation,and the traditional group underwent traditional reduction with hollow screw internal fixation.The surgery time,intraoperative fluoroscopy times,intraoperative guide needle drilling times,failure rate of internal fixation and hip joint function at 6 months after surgery between the two groups were compared.Result:The surgery time of the robot group was shorter than that of the traditional group,and the times of intraoperative fluoroscopy and intraoperative guide needle drilling were less than those of the traditional group,the differences were statistically significant(P<0.05).The failure rate of internal fixation in robot group was lower than that in traditional group,the difference was statistically significant(P<0.05).Six months after surgery,the Harris hip score of the robot group was higher than that of the traditional group,the difference was statistically significant(P<0.05).Conclusion:Orthopedic robot assisted reduction with hollow screw internal fixation in the treatment of elderly abduction and insertion femoral neck fracture has good effect,which can shorten the operative time,reduce the times of intraoperative fluoroscopy and intraoperative guide needle drilling,and is more conducive to the recovery of hip joint function.
