Effect of Hysteroscopic Cold Knife Resection Combined with Levonorgestrel Intrauterine Birth Control System in the Treatment of Patients with Endometrial Polyps
Objective:To investigate the effect of hysteroscopic cold knife resection combined with Levonorgestrel Intrauterine Birth Control System(Mirena)in the treatment of patients with endometrial polyps.Method:A total of 102 patients with endometrial polyps admitted to the Obstetrics and Gynecology Department of Ruijin People's Hospital from September 2021 to June 2023 were selected as the study objects, and they were divided into the control group(underwent hysteroscopic cold knife resection)and the study group(underwent hysteroscopic cold knife resection+Mirena)according to random number table method, with 51 cases in each group.Hemoglobin(Hb),endometrial thickness, estrogen receptors in endometrial tissue[estrogen receptor(ER),progesterone receptor(PR)],serum sex hormone indexes[luteinizing hormone(LH),follicle-forming hormone(FSH),estradiol(E2),progesterone(P)]and inflammatory factors in uterine lavage fluid[reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interferon-γ(IFN-γ)]before and 6 months after operation, complication rate and recurrence of two groups were analyzed.Result:Six months after operation, the complication rate and recurrence rate of the study group were lower than those of the control group, the differences were statistically significant(P<0.05).Six months after surgery, the levels of Hb,LH,FSH and P in both groups were higher than those before surgery, and the levels of endometrial thickness,ER,PR,E2 and inflammatory factors in uterine lavage fluid were lower than those before surgery, and the levels of Hb and LH in the study group were higher than those in the control group, while the levels of endometrial thickness,ER,PR,FSH,P and inflammatory factors in uterine lavage fluid were lower than those in the control group, the differences were statistically significant(P<0.05),but there was no significant difference in E2 levels between two groups(P>0.05).Conclusion:Hysteroscopic cold knife resection combined with Mirena in the treatment of endometrial polyps can regulate the expression of estrogen and progesterone receptors and sex hormone levels, reduce uterine inflammation, and reduce the incidence of postoperative complications and recurrence.
