摘要

目的:分析Mulligan动态关节松动术结合中药熏蒸、牵伸疗法治疗肩周炎的临床效果.方法:选取 2021 年 4 月—2023 年 5 月新余钢铁集团有限公司中心医院康复医学科收治的 90 例肩周炎患者,按随机数字表法分成三组,各 30 例.对照组给予中药熏蒸和牵伸疗法,A组在对照组基础上实施Mulligan动态关节松动术,B组在对照组基础上实施Maitland关节松动术.比较三组疼痛强度、日常生活活动能力、肩关节活动范围和功能.结果:治疗 2 周后,三组Barthel指数(BI)评分和外展、前屈、内旋、后伸活动度及Constant-Murley肩关节评分从高到低依次为A组、B组、对照组,整体疼痛评估量表(GPS)评分从低到高依次为A组、B组、对照组,差异均有统计学意义(P<0.05).结论:Mulligan动态关节松动术结合中药熏蒸、牵伸疗法可以改善肩关节活动范围,有效缓解肩关节疼痛,促进肩关节功能恢复,改善患者日常生活水平.

Abstract

Objective:To analyze the clinical effect of Mulligan dynamic joint loosening surgery combined with traditional Chinese medicine fumigation and stretching therapy in the treatment of scapulohumeral periarthritis.Method:A total of 90 patients with scapulohumeral periarthritis admitted to Rehabilitation Medicine Department of Xinyu Iron and Steel Group Co.,Ltd.Central Hospital from April 2021 to May 2023 were selected and divided into three groups according to random number table method,with 30 cases in each group.The control group was given traditional Chinese medicine fumigation and stretching therapy;the group A was given Mulligan dynamic joint loosening surgery on the basis of the control group;the group B was given Maitland joint loosening surgery on the basis of control group.Pain intensity,activities of daily living,range of motion and function of shoulder joint were compared among the three groups.Result:After two weeks of treatment,the Barthel index(BI)scores,range of motion in abduction,forward flexion,pronation,retroextension and Constant-Murley shoulder joint scores of the three groups were successively group A,group B and control group from high to low,and the global pain assessment scale(GPS)score was successively group A,group B and control group from low to high,the differences were statistically significant(P<0.05).Conclusion:Mulligan dynamic joint loosening surgery combined with traditional Chinese medicine fumigation and stretching therapy can improve the range of motion of shoulder joint,effectively relieve shoulder pain,promote the recovery of shoulder joint function,and improve patients'daily living standards.
