目的:探讨达格列净联合参乌益肾片治疗气阴两虚兼湿浊证慢性肾脏病(CKD)的效果.方法:选取 2023 年 1-6 月景德镇市第三人民医院收治的 100 例气阴两虚兼湿浊证CKD患者作为研究对象,按照随机数字表法分为观察组(予以达格列净联合参乌益肾片治疗)与对照组(予以达格列净治疗),各 50 例.比较两组临床疗效、中医症候(面色少华、神倦乏力、食少纳呆、腰膝酸软)积分、肾功能指标[血肌酐(Scr)、尿素氮(BUN)、24 h尿蛋白定量(24 h UP)、估算肾小球滤过率(eGFR)]、转化生长因子-β(TGF-β)、结缔组织生长因子(CTGF)、Klotho蛋白及不良反应发生率.结果:观察组总有效率高于对照组,差异有统计学意义(P<0.05).治疗 6 个月后,两组中医症候积分、24 h UP、BUN、Scr、IL-12、IL-6、TNF-α均低于治疗前,而eGFR、Klotho蛋白均高于治疗前,且观察组中医症候积分、24 h UP、BUN、Scr、IL-12、IL-6、TNF-α均低于对照组,eGFR、Klotho蛋白均高于对照组,差异均有统计学意义(P<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论:达格列净联合参乌益肾片治疗气阴两虚兼湿浊证CKD具有较为理想效果,有利于改善肾功能和减轻炎症状态.
Study on the Effect of Dapagliflozin Combined with Shenwu Yishen Tablets in the Treatment of Chronic Kidney Disease with Qi and Yin Deficiency Combined with Dampness Turbid Syndrome
Objective:To investigate the effect of Dapagliflozin combined with Shenwu Yishen Tablets in treating chronic kidney disease(CKD)with Qi and Yin deficiency combined with dampness turbid syndrome.Method:A total of 100 CKD patients with Qi and Yin deficiency combined with dampness turbid syndrome admitted to the Third People's Hospital of Jingdezhen from January 2023 to June 2023 were selected as the study objects,and they were divided into observation group(treated with Dapagliflozin combined with Shenwu Yishen Tablets)and control group(treated with Dapagliflozin)according to random number table method,with 50 cases in each group.The clinical efficacy,traditional Chinese medicine syndrome(low facial color,lethargy and fatigue,low diet and lethargy)score,renal function index[serum creatinine(Scr),urea nitrogen(BUN),24 h urinary protein quantity(24 h UP),estimated glomerular filtration rate(eGFR)],transforming growth faction-β(TGF-β),connective tissue growth factor(CTGF),Klotho protein and incidence of adverse reactions between two groups were compared.Result:The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).After 6 months of treatment,traditional Chinese medicine syndrome scores,24 h UP,BUN,Scr,IL-12,IL-6 and TNF-α in both groups were lower than those before treatment,while eGFR and Klotho protein were higher than those before treatment,and traditional Chinese medicine syndrome scores,24 h UP,BUN,Scr,IL-12,IL-6 and TNF-α in observation group were lower than those in control group,eGFR and Klotho protein were higher than those in control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Dapagliflozin combined with Shenwu Yishen Tablets has ideal effect in treating CKD with Qi and Yin deficiency combined with dampness turbid syndrome,which is conducive to improving renal function and alleviating inflammation.
