消化道早癌患者行ESD后接受羟乙基淀粉治疗对营养指标及肠黏膜功能恢复的影响

目的:分析内镜下黏膜剥离术(ESD)后接受羟乙基淀粉治疗对消化道早癌患者营养指标及肠黏膜功能的影响.方法:选取 2022 年 1 月—2023 年 6 月上饶市人民医院收治的 90 例消化道早癌患者,根据随机数字表法将患者分为对照组(行ESD治疗)、联合组(在对照组的基础上给予羟乙基淀粉治疗),各 45 例.比较两组术前、术后 1 周的营养指标、胃肠功能[血清前白蛋白(PA)、白蛋白(ALB)、血红蛋白(Hb)、血清总蛋白(TP)]、肠黏膜屏障功能[血管活性肠肽(VIP)、胃动素(MTL)、胃泌素(GAS)、D-乳酸和二胺氧化酶]、炎症因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、并发症发生率及复发率.结果:术后 1 周,联合组PA、ALB、Hb、TP均高于对照组,差异均有统计学意义(P<0.05).术后 1 周,联合组VIP、MTL、GAS均显著高于对照组,差异均有统计学意义(P<0.05).术后 1 周,联合组D-乳酸、二胺氧化酶水平均高于对照组,差异均有统计学意义(P<0.05).术后 1 周,联合组CRP、IL-6、TNF-α均低于对照组,差异均有统计学意义(P<0.05).两组穿孔、局部复发的发生率比较,差异均无统计学意义(P>0.05);联合组出血发生率为 6.67%,显著低于对照组的 22.22%,差异有统计学意义(P<0.05).结论:消化道早癌患者行ESD后应用羟乙基淀粉治疗的效果显著,可明显改善患者的营养状况,提高肠道黏膜的修复能力,并可降低患者的炎症反应及并发症发生率.
Effects of Hydroxyethyl Starch Therapy on Nutritional Indexes and Recovery of Intestinal Mucosal Function in Patients with Early Gastrointestinal Cancer after ESD
Objective:To analyze the effects of Hydroxyethyl Starch treatment after endoscopic submucosal dissection(ESD)on nutritional indexes and intestinal mucosal function in patients with early gastrointestinal cancer.Method:A total of 90 patients with early gastrointestinal cancer admitted to Shangrao People's Hospital from January 2022 to June 2023 were selected.According to random number table method,the patients were divided into control group(ESD treatment)and combined group(Hydroxyethyl Starch treatment on the basis of control group),with 45 cases in each group.Nutritional indexes,gastrointestinal function[serum prealbumin(PA),albumin(ALB),hemoglobin(Hb),serum total protein(TP)],intestinal mucosal barrier function[vasoactive intestinal peptide(VIP),motilin(MTL),gastrin(GAS),D-lactic acid and diamine oxidase],inflammatory factors[C reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]before and 1 week after operation,incidence of complications and recurrence rate of the two groups were compared.Result:At 1 week after operation,PA,ALB,Hb and TP in the combined group were higher than those in the control group,and the differences were statistically significant(P<0.05).At 1 week after operation,VIP,MTL and GAS in combined group were significantly higher than those in control group,the differences were statistically significant(P<0.05).At 1 week after operation,the levels of D-lactic acid and diamine oxidase in combined group were higher than those in control group,the differences were statistically significant(P<0.05).At 1 week after operation,CRP,IL-6 and TNF-α in combined group were lower than those in control group,the differences were statistically significant(P<0.05).There were no significant differences in the incidence of perforation and local recurrence between the two groups(P>0.05).The incidence of hemorrhage in combined group was 6.67%,which was significantly lower than 22.22%in control group,the difference was statistically significant(P<0.05).Conclusion:The treatment effect of Hydroxyethyl Starch after ESD in patients with early gastrointestinal cancer is significant,which can significantly improve the nutritional status of patients and improve the repair ability of intestinal mucosa.
