目的:探讨多元联动延伸服务模式在肿瘤晚期患者经外周静脉穿刺的中心静脉导管(peripherally inserted central catheter,PICC)管道维护中的应用效果.方法:选取2020年6月—2023年6月景德镇市第二人民医院肿瘤科接受PICC的 80 例肿瘤患者,随机分为对照组(实施以健康教育、疼痛护理、导管维护、饮食指导等为主的常规干预模式)与观察组(对照组基础上联合多元联动延伸服务模式),每组 40 例.记录两组PICC并发症发生情况,对比干预前后导管维护依从性的差异,并随访统计两组患者满意度.结果:干预前,两组导管维护依从性比较,差异无统计学意义(P>0.05);干预后,两组导管维护依从性均升高(P<0.05),且观察组导管维护依从性高于对照组,差异有统计学意义(P<0.05).随访调查结果显示观察组PICC专项护理服务满意度高于对照组,差异有统计学意义(P<0.05).结论:多元联动延伸服务模式用于肿瘤晚期患者PICC管道维护中,能在常规护理基础上增加明显效果,不仅能提高一定的安全性,降低并发症风险,还能从维护依从性入手,提高患者临床配合度,避免不必要的医疗开销,患者满意度也较高.
Application of Multi-linkage Extended Service Mode in PICC Catheter Maintenance of Patients with Advanced Tumors
Objective:To investigate the application effect of multi-linkage extended service mode in the catheter maintenance of peripherally inserted central catheter(PICC)among patients with advanced tumors.Method:A total of 80 patients with advanced tumors who underwent PICC catheterization in the Oncology Department,the Second People's Hospital of Jingdezhen from June 2020 to June 2023 were selected and randomly divided into the control group(routine intervention model mainly consisting of health education,pain care,catheter maintenance,diet guidance,etc)and the observation group(multi-linkage extended service on the basis of routine intervention),with 40 cases in each group.PICC complications in the two groups were recorded.The compliance with catheter maintenance was compared before and after intervention,and satisfaction rates of the two groups were calculated.Result:Before intervention,there was no statistically significant difference in compliance with catheter maintenance between the two groups(P>0.05);after intervention,compliance with catheter maintenance were increased in two groups(P<0.05),and compliance in the observation group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).Follow-up results showed that patients in the observation group were more satisfied with PICC catheter special care services than the control group,the difference was statistically significant(P<0.05).Conclusion:Applying multi-linkage extended service mode in PICC catheter maintenance of patients with advanced tumors increase significant effects on routine nursing and can not only reduce the risk of complications and improve patients'compliance with catheter maintenance,but also avoid unnecessary medical expenses.Besides,patients are satisfied with this mode.
