目的:探讨中国糖尿病风险评分法(CDRS)联合晚期糖基化终末产物(AGE)检查在体检人群糖尿病筛查中的应用价值.方法:选取 2022 年 1 月—2023 年 10 月于萍乡市人民医院进行体检的82 例体检者作为研究对象.所有体检者均行CDRS评估、AGE检查及口服葡萄糖耐量试验(OGTT),以OGTT为金标准,分别对CDRS评分、AGE检查结果及二者联合诊断结果进行一致性检验.而后根据金标准诊断结果将 82 例体检者分为糖尿病组与非糖尿病组,比较两组的CDRS评分、AGE检查结果.使用受试者操作特征(ROC)曲线分析CDRS评分、AGE检查及二者联合诊断糖尿病的临床价值.结果:金标准诊断结果显示,82 例体检者中有 22 例确诊为糖尿病,CDRS评分结果显示有 13 例体检者为糖尿病高危人群,AGE诊断结果显示有11例体检者为糖尿病高危人群,二者联合诊断结果显示有21例体检者为糖尿病高危人群.联合诊断的准确率、敏感度、阴性预测值均高于CDRS评分及AGE检查单独诊断(P<0.05);一致性检验结果显示,联合诊断的Kappa值为 0.905,高于CDRS评分及AGE检查单独诊断的 0.607、0.594.糖尿病组CDRS评分及AGE值均高于非糖尿病组(P<0.05);ROC曲线结果显示,联合诊断预测糖尿病的曲线下面积(AUC)为 0.898,高于CDRS评分及AGE检查的 0.864、0.755.结论:CDRS评分联合AGE检查用于体检人群糖尿病筛查时有着较高的准确率,相较于单独使用CDRS评分及AGE检查诊断,其临床预测价值更高.
Application of CDRS Combined with Advanced Glycation End Product Test in Screening Diabetes in Physical Examination Population
Objective:To explore the application value of Chinese diabetes risk score(CDRS)combined with advanced glycation end product(AGE)test in diabetes screening.Method:A total of 82 patients who underwent physical examination in Pingxiang People's Hospital from January 2022 to October 2023 were selected as the study objects.All subjects underwent CDRS assessment,AGE test and oral glucose tolerance test(OGTT).Using OGTT as the gold standard,CDRS score,AGE test results and their combined diagnosis results were tested for consistency.According to the gold standard diagnosis results,82 patients were divided into diabetes group and non-diabetes group,and the CDRS scores and AGE test results of the two groups were compared.Receiver operating characteristics(ROC)curve was used to analyze the clinical value of CDRS score,AGE test and their combination in the diagnosis of diabetes.Result:The gold standard diagnosis results showed that 22 of the 82 physical examination subjects were diagnosed as diabetes,the CDRS score results showed that 13 physical examination subjects were at high risk of diabetes,the AGE diagnosis results showed that 11 physical examination subjects were at high risk of diabetes,and the combined diagnosis results showed that 21 physical examination subjects were at high risk of diabetes.The accuracy,sensitivity and negative predictive value of combined diagnosis were higher than those of CDRS score and AGE test diagnosis alone(P<0.05).Consistency test results showed that Kappa value of combined diagnosis was 0.905,which was higher than 0.607 and 0.594 of CDRS score and AGE test diagnosis alone.The CDRS scores and AGE values in diabetic group were higher than those in non-diabetic group(P<0.05).ROC curve results showed that the area under the curve(AUC)of combined diagnosis predicted diabetes was 0.898,which was higher than 0.864 and 0.755 of CDRS score and AGE test.Conclusion:CDRS score combined with AGE test has a higher accuracy in diabetes screening in physical examination population,and its clinical prediction value is higher than that of CDRS score and AGE test diagnosis alone.
