腓肠神经营养皮瓣移植在足踝皮肤软组织缺损修复中的应用研究

目的:观察探讨足踝皮肤软组织缺损患者应用腓肠神经营养皮瓣移植修复的效果.方法:选择蚌埠医科大学第一附属医院2021年2月—2024年5月收治的72例足踝皮肤软组织缺损患者,以随机数字表法分为对照组(n=36,给予股前外侧皮瓣移植修复)、观察组(n=36,给予腓肠神经营养皮瓣移植修复).比较两组皮瓣修复效果、并发症发生率、皮肤弹性、美国足踝外科协会(AOFAS)踝-后足评分、温哥华瘢痕量表(VSS)、满意度.结果:观察组皮瓣修复优良率(91.67%)与对照组比较(75.00%),差异无统计学意义(P>0.05).观察组并发症发生率与对照组比较,差异无统计学意义(P>0.05).观察组皮肤弹性优于对照组,差异有统计学意义(P<0.05).观察组治疗后60 d AOFAS踝-后足评分较对照组高,治疗后90 d VSS评分较对照组低,差异均有统计学意义(P<0.05).观察组总满意度较对照组高,差异有统计学意义(P<0.05).结论:足踝皮肤软组织缺损患者应用腓肠神经营养皮瓣移植修复治疗在改善局部皮肤弹性、改善足踝功能、减轻瘢痕遗留的方面具有显著优势,有助于患者获得满意的预后.
Study on the Application of Sural Nerve Nutrient Flap Transplantation in the Repair of Skin Soft Tissue Defect of Foot and Ankle
Objective:To observe and explore the effect of sural nerve nutrient flap transplantation repair in patients with skin soft tissue defect of the foot and ankle. Method:A total of 72 patients with skin soft tissue defect of the foot and ankle admitted to the First Affiliated Hospital of Bengbu Medical University from February 2021 to May 2024 were selected and divided into control group (n=36,given anterolateral thigh flap transplantation repair) and observation group (n=36,given sural nerve nutrient flap transplantation repair) according to random number table method. The repair effect,complication rate,skin elasticity,American orthopaedic foot ankle society (AOFAS) ankle-hind foot score,Vancouver scar scale (VSS) and satisfaction were compared between the two groups. Result:The excellent and good rate (91.67%) of flap repair in the observation group was compared with that in the control group (75.00%),the difference was not statistically significant (P>0.05). There was no significant difference in the complication rate between the observation group and the control group (P>0.05). The skin elasticity in observation group was better than that in control group,the difference was statistically significant (P<0.05). AOFAS ankle-hind foot score 60 days after treatment in observation group was higher than that in control group,VSS score 90 days after treatment was lower than that in control group,the differences were statistically significant (P<0.05). The total satisfaction of observation group was higher than that of control group,the difference was statistically significant (P<0.05). Conclusion:The application of sural nerve nutrient flap transplantation repair in patients with skin soft tissue defect of the foot and ankle has obvious advantages in improving local skin elasticity,improving ankle function and reducing scar left over,which is helpful for patients to obtain satisfactory prognosis.
