Application Effect of Whole Thoracic Oscillation Sputum Extractor Combined with Sequential Sputum Expulsion Plan in Nursing of Patients with Severe Traumatic Brain Injury Undergoing Tracheotomy
Objective:To explore the effect of whole thoracic oscillation sputum extractor combined with sequential sputum expulsion plan in nursing of patients with severe traumatic brain injury (sTBI) undergoing tracheotomy. Method:A total of 72 patients with sTBI who received tracheotomy in the Affiliated Hospital of Yangzhou University from November 2021 to August 2023 were selected and divided into intervention group (n=36) and control group (n=36) by random number table method. The control group was given sequential sputum expulsion nursing,and the intervention group was given whole thoracic oscillation sputum extractor combined with sequential sputum expulsion nursing. The effect of sputum expulsion,sputum viscosity,aggregation degree of sputum,blood gas indexes and complications were compared between the two groups. Result:The daily sputum expulsion volume of the intervention group was more than that of the control group,and the daily sputum aspiration times was less than that of the control group,the differences were statistically significant (P<0.05). After 5 days of intervention,the sputum viscosity of the intervention group was lower than that of the control group,the difference was statistically significant (P<0.05). After 5 days of intervention,the aggregation degree of sputum in the intervention group was lower than that in the control group,the difference was statistically significant (P<0.05). After 5 days of intervention,the arterial partial pressure of oxygen (PaO2) increased and the arterial partial pressure of carbon dioxide (PaCO2) decreased in both groups,the PaO2 in the intervention group was higher than that in the control group,and the PaCO2 was lower than that in the control group,the differences were statistically significant (P<0.05). The incidence of complications in the intervention group[11.11% (4/36)]was lower than that in the control group[30.56% (11/36)],the difference was statistically significant (P<0.05). Conclusion:The application of whole thoracic oscillation sputum extractor combined with sequential sputum expulsion plan in patients with sTBI undergoing tracheotomy can improve the effect of sputum expulsion,improve the viscosity and aggregation of sputum,adjust the blood gas indexes level and reduce the incidence of complications.
