摘要
目的 检索、评价和总结脑卒中患者出院准备度管理的最佳证据,为临床医护人员制定护理实践方案提供参考.方法 检索中国知网、PubMed等国内外数据库,检索时限自建库至2023年7月,由2名循证护理研究者对纳入文献进行质量评价和证据提取.结果 共纳入18篇相关文献,包括2篇指南、9篇系统评价、1篇推荐实践、4篇随机对照试验、2篇类实验研究,文献整体质量较高.总结了入院启动出院准备服务、住院期间健康指导和心理干预、出院前家庭随访准备、出院后持续健康支持4个维度共30条最佳证据.结论 总结了脑卒中患者出院准备度管理的最佳证据,为临床构建提高脑卒中患者出院准备度干预方案提供循证依据.
Abstract
Objective To search,evaluate and summarize the best evidence of the discharge readiness management of stroke survivors,and to provide references for clinical practice.Methods Articles on the discharge readiness of stroke survivors were searched in databases at home and abroad,such as CNKI and PubMed,with the time setting from the establishment until July 2023.The quality evaluation and evidence extraction were carried out by 2 evidence-based nursing researchers.Results A total of 18 relevant articles were included,consisting of 2 guidelines,9 systematic reviews,1 recommendation,4 randomized controlled trials,and 2 quasi-experimental studies.The overall quality of the literature was high.A total of 30 pieces of best evidence were integrated into four dimensions,including the initiation of hospital discharge preparation,health education and psychological interventions during hospitalization,family follow-up preparation before discharge and continuous health support after discharge.Conclusion This study summarizes the best evidence for the management of discharge readiness of stroke survivors and provides evidence-based basis for constructing related intervention programs.
