四阶段式早期康复训练对急性心肌梗死经桡动脉穿刺冠状动脉介入术后患者的应用效果

摘要
目的 探讨四阶段式早期康复训练应用于急性心肌梗死经桡动脉穿刺冠状动脉介入术(percutaneous coronary intervention,PCI)后患者的效果.方法 选择 2020 年 5 月-2022 年 5 月于笔者所在医院行 PCI术的 80例急性心肌梗死患者为研究对象.采用随机数字表法将其分为对照组和观察组,各 40 例.对照组给予 PCI 术后常规护理及康复训练.观察组在术后常规护理的基础上实施四阶段式早期康复训练.比较 2 组首次下床时间、住院时间、心功能指标、生活质量及并发症情况.结果 干预后,观察组首次下床时间早于对照组(t=98.992,P＜0.001),住院时间短于对照组(t=6.545,P＜0.001);观察组左室射血分数和 6 min 步行步距离均高于对照组(t=3.508,P＜0.001;t=8.956,P＜0.001),左心室舒张末期内径低于对照组(t=6.439,P＜0.001);生活质量各维度得分均高于对照组(均P＜0.05);并发症发生率低于对照组(χ2 =4.012,P=0.045).结论 对急性心肌梗死经桡动脉PCI术后患者应用四阶段式早期康复训练,能够有效改善患者心功能,提高其生活质量,降低并发症发生率,有利于患者术后康复.
Abstract
Objective To explore the application of four-stage early rehabilitation among acute myocardial infarction(AMI)patients after transradial percutaneous coronary intervention(TRA-PCI).Methods A total of 80 AMI patients undergoing TRA-PCI in author's hospital between May 2020 and 2022 were selected,and divided into a control group and an observation group,each of 40,according to a random number table.In addition to routine nursing,the control group received conventional rehabilitation,while the observation group underwent four-stage early rehabilitation.Then the first time of getting out of bed,length of hospital stay,cardiac function index,quality of life and complications were compared between the two groups.Results After the intervention,the first time of getting out of bed in the observation group was significantly earlier than that of the control group(t=98.992,P＜0.001),and the length of hospitalization was significantly shorter than the latter group(t=6.545,P＜0.001).The left ventricular ejection fraction,6-minute walking distance and life quality of the observation group were significantly higher than that of the control group(t=3.508,P＜0.001;t=8.956,P＜0.001;P＜0.05),while the left ventricular end-diastolic diameter and incidence of complications were significantly lower than the latter(t= 6.439,P＜0.001;χ2 =4.012,P=0.045).Conclusion The application of four-stage early rehabilitation training is effective among AMI TRA-PCI patients,as it can promote their cardiac function,life quality and postoperative recovery,and lower the incidence of complications.
